本研究は, 高齢者の腹膜透析(CAPD)療法導入に至る現状を, インフォームド・コンセント(IC)と心理状態に注目し, 分析した.CAPDによる透析者700人に調査用紙を配布し, 427人から回答を得た.有効回答は410で, CAPD導入時年齢が65歳以上の「高齢群」(N=74)と65歳未満の「若年群」(N=336)の2群で比較検討した.その結果, 2群に共通して, 7割前後の者が透析療法導入について自己決定をしていたが, 透析に踏み切った段階でもなお『受容』に至っていない者が少なくなかったことが明らかになった.また, 若年群との比較における高齢群の特徴として, (1)合併症を有する者が多いこと, (2)緊急透析導入が少なくないと推察されること, (3)半数の者がCAPDの選択において自己決定をしていたが, 消極的理由での選択が多かったこと, (4)CAPDの選択後も様々な不安を抱えている者が多かったこと, が明らかになった.更に以上のような状況は, 透析療法導入に向けたICのあり方(内容や方法, 時期)にも一因があることが推察された.今後の課題として, 円滑な透析療法の導入および導入後の安定的な生活を維持するための, 保存期から導入期に至る継続的なアプローチのあり方についての検討の必要性が認識できた. |

This study was done to study the emotional responses seen in elderly patients with end stage renal diseases, who are undergoing continuous ambulatory peritoneal dialysis (CAPD) treatment. Information was obtained by questionnaire from 410 patients undergoing CAPD. Patients were divided into two groups, those who were less than 65 years old at initiation of CAPD and those were older than 65. The mental, emotional, and psychological changes, pre- and post- therapy, were analyzed and compared in these two groups, in relation to the knowledge and information these patients had received. 70% of the patients had decided by themselves to accept the CAPD treatment, and the remainder had had doubts about receiving treatment. Half of the elderly group had selected CAPD by themselves, based on negative reasons, feeling anxiety at the initiation of CAPD. From the results of this study, it would be important, from a nursing standpoint, to make an effort to give more therapeutic information to elderly people early on. The proper procedures for obtaining informed consent from the elderly should be given further study. |
