| 重症心身障害児（者）（以下，重症児（者）とする）は，言語による意思の伝達が困難な場合が多いが，重症児（者）の表出するサインに気づきケアに活かすことのできる看護師がいる．本研究では，看護師が行っている重症児（者）の意思の確認方法を文献から明らかにすることを目的とした．医中誌Web 版（Ver.5）を用いて文献検索を行い，10件を対象文献とした．対象文献を精読し，意思の確認方法が記述されている文章を抽出し分析した．その結果，【関わりの積み重ねからの解釈】【相互作用に基づいた探索】【意思を理解しようとする姿勢】【客観的な指標を用いた判断】【他者との検討と共有】の5カテゴリーが抽出された．看護師は，重症児（者）の意思を理解しようとする姿勢を基盤に持ち，関わりの繰り返しから得られた情報を，意思の確認に活かしていた．また，重症児（者）からの反応や非言語的な表出を重症児（者）の意思と解釈し，それらについて他者との検討と共有を行い，より確証の持てる確認方法としていた．検討と共有を繰り返すことは，重症児（者）看護経験の浅い看護師にとっても，重症児（者）を理解することの一助となり看護実践に活かすことができる．今後は，検討と共有の具体的な方法を明らかにする必要性が示唆された． | ||||||||||

| Although patients with severe motor and intellectual disabilities often have difficulty expressing their intentions verbally, there are nurses who are able to recognize their intentions and utilize them in their care. This study sought to determine from the literature the methods used by nurses to confirm the wishes of patients with severe motor and intellectual disabilities. A literature search was conducted using the web version of the medical journal, and 10 references were selected as target references. The target literature was carefully read, and sentences describing the method of confirming intentions were extracted and analyzed. As a result, the following five categories were extracted [Interpretation based on the accumulation of involvement], [Exploration based on interaction], [Attempts to understand intentions], [Judgment using objective indicators], and [Consideration and sharing with others]. The nurses had a foundation of trying to understand the intentions of the patients with severe motor and intellectual disabilities, and used the information obtained from repeated interactions to confirm their intentions. The results were reviewed and shared with others to provide a more reliable method of confirmation. The need to clarify specific methods of review and sharing was suggested. | ||||||||||
