| 本研究は，X法人の重症心身障害児（者）施設全6施設「以下，（6施設）」を対象に，2015年4月1日から2020年3月31日までの過去5年間の入退所の実態を明らかにすることを目的とした．調査方法は質問紙を用いた郵送調査を実施し，6施設全てから回答を得ることができた（回収率100%）．本論文では，新規で入所した重症心身障害児（者）「以下，（新規入所児者）」および退所した重症心身障害児（者）「以下，（退所児者）」の実態の2つを軸に分析をすすめた．その結果，新規入所児者が184人，退所児者が 119人となった．年齢別では，新規入所児者は「0-2歳」が32人（17.4％），退所児者は「3-5歳」が14人（11.8％）と最も高い割合となった．また，年齢によって入退所の理由に違いがみられた．本調査の結果から， 新規入所児者では，入所する本人の事由よりもその家族が抱える課題を理由に入所するケースが多く見受けられた．そのため，入所する本人のみを支援するのではなく，同時にその家族も含めて支援をすることがより重要と考える．退所児者では，看取りを含めた死亡退所がおよそ4割を占めていた．このことから，入所している本人やその家族が望む生活をより的確に実現することができるように， 普段の施設生活において，入所している本人のアセスメントを深め，そこで得られた情報を積み重ねていくことが重要と考える． | ||||||||

| The purpose of this study is to clarify actual situation of upon admission and discharge in a total of 6 facilities for persons with severe motor and intellectual disabilities. In this paper, the analysis was based on two points: actual situation of new inpatients with SMID (SMID: severe motor and intellectual disabilities), and the actual situation of discharged patients with SMID. As a result, the totals from the 6 facilities were 184 new inpatients with SMID, and 119 discharged patients with SMID. The highest percentage among new inpatients with SMID was 32 (17.4%) of patients“0-2 years old”, and among discharged patients with SMID 14 (11.8%) of patients“3-5 years old”. In addition, it became clear that the reasons for upon admission and discharge differ depending on the age. This finding indicates new inpatients with SMID need not only care but also support for their family. For discharged patients with SMID, it is important to realize their needs during the period of hospitalization. | ||||||||
