| 介護施設の利用者は，加齢や障害などによる身体機能の低下や構造的な変化により車椅子での生活となり，姿勢保持・姿勢調節機能低下や嚥下能力低下などが危惧される．今回，介護施設利用者の食事場面の姿勢に関してアンケート調査を実施し，摂食嚥下時の姿勢の現状と課題の検討を行った．結果，問題姿勢は普通車椅子，リクライニング車椅子，ベッド上が多かった．姿勢の崩れの問題は滑り座位，斜めすわりが多く，原因は体幹・頸部の緊張，筋力の低下，脊柱の可動性低下，ポジショニン グ不良によるものと考えられ，姿勢コントロールが重要と考えた．日々の誤嚥リスクの課題が多く挙げられ，今後，多職種・医療機関との連携を図り，高齢者の食事姿勢の向上に繋げ，安全な食事ができる体制づくりが必要と考えた． | ||||||||||

| A user of a nursing care facility lives in a wheelchair due to a decrease in physical function or structural change due to aging or disability, and there is a concern that the posture maintaining / adjusting function and the swallowing function may decline.In this study, we conducted a questionnaire survey on the posture of the nursing facility users in the eating site, and examined the current state and issues of the posture during swallowing. As a result, the problem postures were ordinary wheelchairs, reclining wheelchairs, and beds. The problems of postural collapse were most often caused by sliding sitting and oblique sitting. The causes were thought to be trunk / neck tension, decreased muscular strength, decreased mobility of the spine, and poor positioning. There were many issues of daily aspiration risk, and we thought that it would be necessary to cooperate with various types of occupations and medical institutions, improve the eating posture of the elderly people, and create a system that enables safe eating. | ||||||||||
