| 本研究の目的は，予期せぬ死を遂げた患者の遺族が抱く救急外来の医療者へのニーズを明らかにし， 救急外来における家族へのケアを促進するプロトコル作成への一助とすることである．研究方法は， 遺族13名を対象に，救命処置時，死亡宣告時，死亡宣告後，死亡帰宅時，死亡帰宅後の各場面の医療者へのニーズに関して半構造化面接を実施し，質的帰納的に分析を行った．結果，家族は，救命処置時には，処置を優先しながら他職種と連携を図り，家族ケアを求めていた．そして，死亡宣告後には， 家族の情緒的支援に重点を置きながらも，帰宅に向けての手段的支援を求めていた．死亡帰宅後は， 関わった医療者からの継続的な支援を求めていた．国内での遺族ケアの実施率は低く，死亡帰宅後までの支援は実施できていない現状がある．今後，本調査で明らかになった遺族のニーズを活かしたプロトコルの作成を実現させたい． | ||||||||||

| This study aimed to identify the needs for emergency department services among the bereaved who experienced an unexpected death of a family member, to develop a protocol to promote family care practiced by the emergency department. Semi-structured interviews were conducted in 13 bereaved families to investigate their needs for healthcare services during emergency medical procedures, at the time of and after confirmation of the patient’s death, and at the time when and after the body is returned to home. The obtained data were analyzed using a qualitative and inductive approach. As a result, the families, while placing a priority on patient’s care during emergency procedures, wanted to receive care for themselves performed in collaboration with other professionals. After the confirmation of death, the families wish to receive practical support for returning the body home, while also focusing on their emotional support. After returning home, they sought continuous support from the healthcare providers involved. The implementation rate of bereavement care in Japan is low, and no support has been currently provided after a body is returned to home. In the future, we would like to develop a protocol that reflects the needs of the bereaved families identified in this study. | ||||||||||
