| 本研究は，重症心身障害児（者）施設（以下，「重症児者施設」）を対象に，重症心身障害児（者）（以下，「入所児者」）の実態およびアセスメントの現状と課題を明らかにすることを目的とした．全国に展開する X 法人の重症児者施設全6施設「以下，（6施設）」を対象に，質問紙を用いた郵送調査を実施し，6施設から回答を得ることができた（回収率100%）．本論文では，「施設で重視しているアセスメント項目」を軸に，施設別，分類別，ICFの概念図の使用，の3点から分析をすすめた．その結果， 施設別では，6施設中4施設が「医療」に関する項目を最も重視していた．分類別では，「医療」や「身体機能」に関する項目を多く重視していた．ICF の概念図を用いて整理をすると，6施設全体では， ICFの各構成要素において項目数の違いはあったものの，どれかの構成要素に偏ることなく重視している傾向にあった．これらの結果は，他の障害者支援施設にはない，重症児者施設特有の結果であると考える．全体のまとめとして，6施設におけるアセスメントの現状は，主に「医療」や「身体機能」 に関する項目が重視されている．加えて，入所児者を総合的に理解すること，地域とのつながりを構築すること，個別性に関する項目を重視すること，これら3つの課題が明らかとなった． | ||||||||

| The purpose of this study is to clarify actual conditions of patients with SMID (SMID; severe motor and intellectual disabilities) and current status and issues of assessment for facilities for persons with severe motor and intellectual disabilities. Focusing on the“the assessment items emphasized in facilities”, the analysis was based on three points: by facility, by category, by using a conceptual diagram of ICF (ICF; International Classification of Functioning, Disability and Health). As a result, by facility, 4 of the 6 facilities had emphasized the assessment items related to“medical”, by category there were many of the assessment items related to“medical”and“physical function”, by using a conceptual diagram of ICF each assessment item was perceived equally. In summary, as for the current status of assessment at the six facilities for persons with severe motor and intellectual disabilities, items related to“medical”and“physical function”are mainly emphasized. In addition, comprehensive understanding of the patients with SMID, building connections with the community, to emphasize on items related to individuality, three issues were clarified. | ||||||||
