| 中華人民共和国は建国（1949年）後，計画経済体制の元，医療保障システムが全く整っていない状況からスタートした．当時は，戸籍制度の確立に伴って，都市部において労働保険医療制度及び公費 医療制度が実施された . つまり都市部のみ限定的に「国民皆保険」がほぼ実現されていたといえよう． 一方，農村部では互助共済の農村合作医療制度が設立され，「発展途上国で医療費問題を解決できる 唯一の模範例であり，成功した医療革命」と称賛され，国際社会から注目されるようになった．1978 年に「改革・開放」政策が提出され，中国は市場経済へと移行し始めた．それとともに，従来の医療 保障システムが崩壊してしまった．1990年代から，中国の経済は急成長に入り，地域モデルの試行を 通じて，都市部には従業員基本医療保険制度，農村部には新型農村合作制度が実行された．それに引 き続いて都市部住民基本医療保険制度が構築され，全民保険（日本の国民皆保険に相当）は制度上に おいて確立された．しかし，現制度執行以来，制度間の格差，医療費の高騰などの問題が顕在化して きた．今後，経済成長が鈍化する可能性があることをふまえると，少子高齢化に関する問題を直視し， 医療保障システムの充実化と新たな社会福祉制度の整備を求めて行く必要がある． | ||||||||

| After the establishment of the People's Republic of China, a planned economic system was established and the medical security system was newly created from scratch. Because of the household registration system, there were different insurance systems between urban and rural areas. These insurance systems played an active role in increasing the public health level and economic growth of those decades at the time. Since "Reform and Openness Policy" was submitted in 1978, China began to shift to the market economic system. The original medical security system failed to adapt to the market economic system, and was brought to the brink of collapse. Since the 1990s, China has entered the period of high-speed development of economy. The medical insurance system had to reform. Through the pilot work, the Urban Employees’Basic Medical Insurance Program, the New Rural Cooperative Medical Scheme, and the Urban Resident Basic Medical Insurance Program were built one after another. The national medical insurance system of China has been achieved institutionally. In this paper, we will explain the whole picture of China's public medical care system through the reform of the economic system. Based on that, we would sort out problems existing in the operation of the medical insurance system and find the solution in the future. | ||||||||
