| 手術療法を受けた患者の退院後の日常生活への適応支援について検討するために,全国の300床以上の病院234を無作為に抽出し,外科系病棟看護師長を対象に調査を行った.内容は,退院指導や病院内および地域における保健医療福祉職の連携に関する看護師長の認識である(回収率53.8%).退院指導については,継続看護部門や患者相談室が未設の病院より既設の方が,術後の治療や生活上の障害に適応するための指導をしていると認識していた.病院内の職種連携については,患者相談室が既設の病院より未設の方がその必要性と連携上の問題が多いことを認識していた.病院と地域の保健医療福祉職の連携については,退院計画・調整部門の既設の方で連携が進んでいた.その場合,患者の承諾を得ないで個人情報を提供している割合が多かった(p<0.01). 以上の結果から,退院指導や病院と地域の保健医療福祉職の連携や,継続看護部門など病院によっては設備環境の整備状況が不備であり,術後患者の退院後の受け入れ体制はシステムとして十分には整備されていない病院が多いことが明らかになった.術後患者が,生活上の障害を受容して自己管理ができるような行動を育む適応支援を行うためのシステムの必要性が示唆された. | ||||||||||

| To clarify the adjustment supports of post-discharge patients who received operative treatment, 234 hospitals were randomly selected from a nationwide list of hospitals with 300 beds or more, and a questionnaire survey was conducted with the heads of surgical nursing staff. The contents were about discharge education and cooperation with health medical welfare staffs in hospitals and communities (response rate 53.8%). Discharge education indicated hospitals that have continuing nursing sections and patient clinic rooms where guidance is given in order to adjust to the handicap of post-operative treatment and life. Cooperation between staffs in the hospital recognized that there are many problems in the hospital when patient clinic rooms don't exist and cooperation is minimal. As for cooperation with hospital and health medical welfare staffs in the community, cooperation was advanced with the hospitals existing discharge plan. In that case, there were a lot of ratios to provide individual information without obtaining the patient's consent (p<0.01). From the above results, there were problems in discharge education and in cooperation of hospital and health medical welfare staffs in communities, and it was clarified that the preparation for post-operative patient's reception was not well established. The necessity of adaptive support for post-operative patients whose lives will be permanently altered, both mentally and physically was suggested. | ||||||||||
