| 目的 介護保険法の基本理念は,「利用者の保護」「個人の尊重」「自立支援」と規定されている.本研究では,介護保険の理念のひとつ「自立支援」に着目した.自立支援を医療関係者は,日常生活動作(ADL)の維持拡大とし,福祉関係者は,「自己決定の行使」「生活主体者として生きる」及び「社会参加」としてとらえている.高齢者や援助者の寝たきり高齢者への社会参加を支援しようとする意識を明らかにすることを目的とした.方法 寝たきり高齢者の代表的な事例を示し,自立支援として何を優先して支援しようとしているのかを,K市の高齢者(56人),看護師(284人)及び福祉職(160人)を対象に調査した.結果 高齢者は,リハビリテーションによる歩行,車椅子移乗訓練や排泄訓練を71.4%の者が希望していた.自己決定の行使や社会参加を希望する者は,7.1%と少数であった.看護師は,家族の介護負担軽減を優先する者が58.1%と最も多く,次いでリハビリテーションによる身体の機能訓練であった.福祉職は,自己決定の行使や社会参加の支援,家族の介護負担軽減,リハビリテーションによる機能訓練を同等に選択していた.自己決定の行使や社会参加を支援しようとする意識は,福祉職が高齢者,看護師より有意(p<0.01)に高かった.特に自己決定の行使や社会参加を支援しようとする意識は,社会福祉士や介護福祉士が高く,「ユニットケア」「グループホーム」を取り入れている施設職員により高い傾向があった.結論 寝たきり高齢者の自立支援は,高齢者,看護師,福祉職で違いがあった.寝たきり高齢者の社会参加を支援しようとする意識は,先進的な介護を取り入れている施設から進展いくと推察された. | ||||||||||

| The Long-Team Care insurance Law specifies three basic philosophies: protection of the individual, respect for the individual and self-care. The present study focuses especially on self-care. Health care practitioners recognize self-care to be preserving and improving activities of daily life (ADL), whereas welfare professionals consider it to be self-determination, independent living and social participation. We could find no report on bedridden elderly people's social participation. The present study was conducted on 56 normal elderly people, 284 nurses and 160 welfare professionals in K City to investigate what is the priority issue with regard to self-care in typical bedridden elderly people. We found that 71.4% of normal elderly people liked rehabilitation that wed walking, pushing a wheelchair and toilet training but only 7.1% desired self-determination and social participation. Nurses gave priority first to reduction of the care burden on the families (58.1%) and then physical function rehabilitation training. Welfare professionals selected self-determination, support of social participation, reduction of the care burden on the families and physical function rehabilitation training equally. Welfare professionals were significantly more interested in support of self-determination and social participation than normal elderly people and nurses (p<0.01). Social welfare counselors, welfare careworkers and in particular and staff at institutions employing the unit care or group home system are more interested in support of self-determination and social participation. Regarding support of bedridden elderly people's self care, there are differences among normal elderly people, nurses and welfare professionals. It was suggested that institutions providing advanced care services would promote the attitude toward support of bedridden elderly people's social participation. | ||||||||||
