| 特別養護老人ホーム(特養)において常勤医配置前の3年間と配置後の3年間の入所者の実態や入所者, 職員の意見などから, 常勤医配置がもたらす医療福祉的効果についてを検討した.岡山県A園(入所定員110人)の平成5年から10年度まで6年間の入所者全員を対象者として調査した.実態についてはカルテを中心に死亡診断書等を参考にまとめた.常勤医配置により外来受診, 入院日数, 入院数の減少, 投薬人数や投薬薬剤数の減少, 死亡者の高齢化, 施設内死亡の増加, 発症後24時間以内死亡の減少, 心疾患死亡の減少, 老衰の増加などの成果がみられた.特養は生活支援の場所ではあるが, 入所者の重度化, 高齢化はますます進み, さらに, ターミナルケアを実施する施設が増加しており, 特養における医療のはたす役割は大幅に増している.福祉施設の特徴を生かしながら, 特養に常勤医配置やオープンシステムの導入などが望ましいケアを実施するための大切な条件になろう. | ||||||||

| The effectiveness and significance of assingning a regular staff physician to a special nursing home for the elderly were studied by reviewing the actural conditions and opinions of residents and other staffs regarding the periods of three years respectively before and after the assigment. Subjects were all Residents at home A in Okasyama prefecture (capacity : 110person) between 1993 and 1998. the actual conditions were studied by perusing medical records and referring to the deaths certificates, ect. The results revealed that the assignment of a regular staff physician reduced the number of the outpa-tient clinic visits and those hospitalization, the duration of hospital stays and the number of those given medication and of the kinds of medicatins, elevated the age of the dead, increased the numbers of deaths at the home, however, decreased the numbers of deaths within 24 hours after the onset of illness and of deaths by heart disease, and, increased the incidence of natural death. While the Special Nursing Home serves to support the life of residents, the severity of illness and aging of the residents are intensifying. there is an increasing number of homes giving terminal care, indicating that the role of medical services at Spcial Nursing Homes is greatly expanding . It is essntial for care of the aged that regular staff physician are assigned and features of a welfare facility retained. | ||||||||
