| 慢性期の患者や高齢者で重複障害を抱える患者に対して, どのような言語訓練・援助システムを確立すべきかを目的として, 岡山西大寺病院で言語訓練を行った患者17例の結果をまとめ, 考察を加えた.17例は, 実施した言語訓練の内容によって主として音声・構音訓練を実施したもの6例(音声・構音訓練群)・主として失語症訓練を実施したもの2例(失語症訓練群)・主として実用的コミュニケーション訓練を実施したもの9例(実用的コミュニケーション訓練群)の3群に分類し, 意識障害・非言語的知能低下・失語症状・構音障害の有無について, 初回評価時(言語訓練前)と最終評価時(言語訓練後)での変化を検討し, 以下のような結果を得た.1.音声・構音訓練群及び失語症訓練群に対しては, 従来の刺激法を中心とした訓練で症状は改善した.,2.実用的コミュニケーション訓練群に対しては, 今回行った訓練内容では改善が得られなかった.この群は意識障害が最大の問題であり, チームアプローチ・家族指導も含めて, 新たな言語訓練プログラムと援助システム確立の必要性が考えられた. | ||||||||||

| To establish a language training and support system for chronic patients or aged and multiple-handicapped patients, we investigated seventeen patients who had received language training at Okayama Saidaiji Hospital. Seventeen patients were classified into three groups : six patients received mainly voice or articulation therapy; two patients received mainly aphasia therapy; nine patients received mainly pragmatic communication therapy. We compared the degrees of disordered consciousness, nonverbal intelligence disorders, aphasic symptoms and articulation disorders between the first assessment (pre-speech therapy) and the last assessment (post-speech therapy), so we found the following results.1 . For the voice or articulation therapy group and aphasia therapy group, the therapy based on the ordinary stimulation method was effective. 2. For the pragmatic communication therapy group, our therapy program was not effective. We recognized that disordered consciousness was the main problem of this group, and we discussed the need for establishing a new language training program and support system including team approach and consultation with family. | ||||||||||
