| 初診時に非言語性知能の重度低下に加えて全失語を認めた患者1例を対象に,段階を踏んだ訓練プログラムを実施した。その結果,全失語は改善しないという従来の知見に反して比較的良好な改善過程を示した。本研究ではその改善過程をまとめ,全失語患者に対する言語訓練プログラムのあり方を考察した。実施した訓練と結果は以下の如くであった。第1段階 : 非言語性知能を主とした認知能力の改善訓練とモーラ指折り法による絵単語の聴覚的理解と文字理解促進訓練を行った。その結果,非言語性知能・絵単語の聴覚的理解・かな文字理解力すべてが改善した。第2段階 : かな文字単語を利用して復唱・音読・呼称訓練を行った。その結果,単語(絵・文字とも〉を見ての音節の分解・抽出, かな文字認知力が改善した。第3段階 : 絵単語と文字単語を併用してトーキングエイドを用いてかな文字を操作する訓練を行った。その結果,トーキングエイドでの文字操作(絵を見て文字を打つ・質問に対して頻用することばを文字で応答するなど)が可能となり,訓練したことばは絵単語を見ると自発的にかな文字単語として書けるようになった。本例では文字単語を習得するために必要と考えられる非言語的認知能力,聴覚的・視覚的理解力,単語の音節の分解・抽出に留意した段階を踏んだ訓練が有効であり文字言語が単語レベルで操作できるまでになった。その際媒介としてのトーキングエイドの使用が効果的であった。 | ||||||||||

| We executed the small-stepped training program for an patient who showed severe nonverbal intelligence disorder and global aphasia at intake time. The patient showed relatively good recovery. We reported the recovery process and discussed the problem of a language-training program. The 1st step : The training for cognitive ability (nonverbal intelligence) and the training for auditory and visual comprehension of words by the method "mora segmentation by fingers" produced the recovery of performance IQ and the reading ability of "kana" characters. The 2nd step : The training of repetition, reading aloud and naming with the cues of "kana" characters produced the recovery of ability of analysis and extract of syllables and the recognition of "kana" characters. The 3rd step : The training of "kana" character operation with the Talking Aid (TA) produced TA operation ability (naming the drawings by letters and answering to the questions with frequent words) and writing ability of "kana" characters. The small-stepped program (for recovery of nonverbal IQ and ability of auditory and visual comprehension, analysis and extract of syllables, and recognition of "kana" characters) was effective, Especially, use of TA was effective. | ||||||||||
