| 迂回反応が多くみられた健忘失語の1例に対し喚語訓練を行った。線画カードを提示し,喚語できない時は迂回するよう教示し,それでも喚語できない時は迂回的ヒントを提示した。結果は以下のようであった。1.訓練終了後,訓練語では即時正答数が有意に増加したが,非訓練語では有意に増加しなかった。2.即時正答が増加するにつれ,迂回反応は減少していった。3.迂回反応とヒントの両方で正答に至る傾向が高くみられたのは,形態・属性に関する叙述であった。4.喚語の改善に伴い,仮名自発書字も改善した。以上の結果から,迂回反応を用いた訓練法が有効であることが示された。また,単語の視覚イメージの強化が喚語の改善に有効と思われた。さらに仮名書字障害の改善から,迂回反応のメカニズムについて考察した。 | ||||||||||

| Naming training was performed in the patient with amnesic aphasia who produced many circumlocutions. Line drawing cards were presented to the patient and the patient was instructed to produce circumlocutions as possible when he could not name the object. Circumlocution hints were presented when the patient could not name the object by using his own circumlocutions. Major findings obtained in this study were as follows : 1. The performance of trained words increased significantly on the post-training test; however, that of the non-trained words did not increase on the post-training test. 2. The immediate correct response increased while the circumlocution decreased. 3. Circumlocutions and hints about shape and attribute were the most effective in naming. 4. Writing ability of kana characters improved with the recovery of naming difficulty. These results suggested that use of the circumlocution was effective on the naming training and the reinforcement of the visual image of word was also effective in improvement of naming difficulty. The mechanism of circumlocution was discussed through the improvement of kana character writing. | ||||||||||
