本研究は,訪問看護ステーションに勤務する看護師299名を対象に,利用者を尊重した視点での看護実践評価尺度の作成と,それによる訪問看護師の看護実践の評価を目的とした.訪問看護師の看護実践評価尺度は,7因子25項目からなる二次因子モデルを仮定し,その因子モデルへのデータの適合度を確証的因子分析により分析した.結果,CFI=0.910,RMSEA=0.061で,いずれのパス係数も統計学的に有意水準にあり,本尺度の構成概念妥当性は検証できた.またCronbachのα信頼性係数は,尺度全体は0.865,各因子は0.694〜0.846であり,尺度の信頼性は概ね良好であった.訪問看護師の看護実践の項目平均値は「看護師間の協働」が最も高く,「看護実践の達成感」が最も低い傾向であった.また,所長とスタッフの比較では,所長の平均値,スタッフの訪問看護師経験年数3年未満と3年以上の比較では,3年以上の平均値が高かった(p<0.05).スタッフの正職員とパート職員の比較では,平均値に有意差はなかった. |

The purposes of this study were to develop a scale for evaluating visiting nurses' practice of client-centered care (care that respects the client), and to evaluate their actual nursing care practice, using the developed scale. Study subjects comprised 299 nurses working at home-visit nursing stations. For a scale to be used in evaluating visiting nurses' care practice, we developed a second-order factor model comprising seven factors and 25 items. We then analyzed the fitness of the data to the factor model, via confirmatory factor analysis. The comparative fit index (CFI) was 0.910; the root mean square error of approximation (RMSEA) was 0.061. Both path coefficients were statistically significant, and the scale's construct validity was deemed acceptable. Scale reliability was satisfactory (Cronbach's alpha was 0.865 for the whole scale, 0.694 - 0.846 for each factor). Regarding visiting nurses' evaluation of their care practice, of the seven factors, "cooperation among nurses" was rated highest tendency, and "sense of accomplishment from care activities" was rated lowest tendency. Comparison of evaluation scores between managers and staff nurses showed that manager scores were higher; comparison of evaluation scores between nurses with less than three years' home-visit nursing experience and those with 3 or more years of such experience showed that the nurses with 3 or more years of experience scored higher (p<0.05). Comparison between full-time nurses and part-time nurses disclosed no significant difference. |
