双子を育てる母親の負担は大きく,虐待発生率が一般の8〜10倍であることも報告されている.本研究は双子の母親10人を対象に半構成面接を実施し,育児ストレスと今後の支援のあり方を明らかにした.出産〜4か月までの育児ストレスは,【双子育児に対する指導への戸惑い】【睡眠・休息の不足】【双子への授乳の困難さ】【双子育児への負担】【双子を連れての外出の困難さ】【ソーシャルサポートの不充分さ】【発達への不安】【上の子に十分対応できない申し訳なさ】が抽出された.5か月〜12か月では,【思い通りにならない双子育児の負担】【離乳食への負担】【自分の時間の確保困難】が追加された.児の定頸という成長・発達に伴う育児の変化が育児ストレスに影響していることから,定頸までの支援の重要性が示唆された.助産師は,母親が負担を軽減できる双子育児の実践的な指導と退院後に確実に地域支援が受けられる橋渡しを行うことが早急な課題といえる. |

Mothers of twins bear a greater parenting burden. It has been reported that the rate of child abuse by them is 8 to 10 times higher than by the general population. This study examined their parenting stress and appropriate support for them through semi-structured interviews with 10 mothers of twins. As a result, the following categories were extracted as forms of parenting stress they experienced after delivery until 4 months: [Confusion caused by twin parenting advice]; [insufficient sleep and rest]; [difficulty in breastfeeding twins]; [a greater burden of parenting twins]; [difficulty in going out with twins]; [insufficient social support]; [anxiety over developmental issues]; and [feeling sorry for insufficient care for elder children]. In addition to these, five to 12 months after delivery, they experienced: [the burden of troublesome twin parenting]; [burden of feeding twins baby food]; and [difficulty in finding time for themselves]. These results suggest the importance of support for these mothers until their twin' s necks become stable, as their parenting stress is associated with developmental changes; therefore, it is an urgent issue for midwives to establish a system facilitating the provision of practical twin parenting advice and community support for mothers after discharge. |
