ICD-10コードとDPC コードの関係については，診療内容と不合致なICD-10コードの選択が議論となっており，ICD-10コードにおいては「詳細不明コード」（いわゆる「.9」）の選択が多い施設には診療報酬上のペナルティーが科せられている．また，DPC/PDPS 対象施設の条件にも詳細不明コード付与割合が全退院患者数割合で10% 以下が条件となっている．本研究では岡山県の医療機関から収集したDPC データから「肺炎」症例を対象に「J18.9：肺炎，詳細不明」コード付与症例における様式1（患者属性や病態等の情報）データ入力項目とその関係性を明らかにすることを目的とする．先行研究においては「J18.9：肺炎，詳細不明」コードの使用が減少していることが報告されていたが，本研究の検証では「J15.9：細菌性肺炎，詳細不明」など，他の詳細不明コードに分散されている可能性が示唆された．DPC データにおける様式1には，肺炎患者の詳細な症状評価のデータは収載されているが，詳細不明コードを解消するための評価データの収載は無かった．また，F ファイルデータ（行為明細情報）から入院時の検査実施項目の抽出は可能であるが，その検査所見についてはDPC データのみでは不明である．今後はこれらのデータセットにも，ある程度の臨床的な所見情報の収載の検討が必要である． |

Regarding the relationship between ICD-10 codes and DPC codes, the selection of ICD-10 codes that are inconsistent with the content of medical care has been controversial, and facilities with a large number of “.9”, unspecified codes, in ICD-10 codes have been penalized in terms of reimbursement. In addition, the condition for facilities to be included in the DPC/PDPS is that the percentage of“ unknown details” codes must be less than 10% of the total number of patients in the unit. The purpose of this study is to clarify the relationship between Form 1 data entry items and “J18.9: Pneumonia, unspecified” codes in “pneumonia” cases from DPC data collected from medical institutions in Okayama Prefecture. In a previous study, it was reported that the use of“ J18.9: Pneumonia, unspecified” codes was decreasing, but the present study suggested that the codes may be distributed among other undetailed codes such as “J15.9: Bacterial pneumonia, unspecified” codes. Although Form 1 (information on patient attributes and conditions) in the DPC data contained data on detailed symptom evaluation of pneumonia patients, it did not contain any evaluation data to resolve the missing-detail codes. In addition, although it is possible to extract laboratory tests performed at the time of admission from F-file data (action description information), the laboratory findings are not known from the DPC data alone. In the future, it is necessary to consider the inclusion of some clinical findings in these data sets. |
