| 病棟看護師の退院支援が適切であったかの評価は，病棟看護師に十分に還元されづらく，改善が困難な一面がある．本研究では，病棟看護師の退院支援について，訪問看護ステーションの訪問看護師からの評価を得ることで，退院支援の効果や改善点を明確化することを目的とした．A 県の訪問看護ステーション177か所に勤務する訪問看護師531名を対象に無記名自記式質問紙調査を行った．回収数は208件，回収率は39.2% であった．訪問看護師退院支援満足度10段階の評定尺度では，1～5の低群が119名（60.4%），6～10の高群が78名（39.6%）であった．ロジスティック回帰分析の結果，退院支援満足度に関連する要因は「退院後の生活に関する情報収集」（OR：5.861），「病院との地域連携」（OR：5.185），「サービス利用者から退院支援への要望や苦情を受けた経験」（OR：2.351），「訪問看護師経験年数」（OR：1.004）の4項目であった．また，退院支援内容では，退院後の注意点や医療処置の内容等は半数以上が十分としていたが，具体的な介護の方法や生活面の不安等の支援は不十分と評価していた．病棟看護師は，退院後の生活環境や介護方法も意識し患者に関わっていくことで，退院支援の質向上が期待できる． | ||||||||||

| Discharge support work performed by ward nurses is difficult to evaluate and improve in the current situation where patient information after discharge is limited. The purpose of this study was to clarify the evaluation and improvement points of discharge support by ward nurses based on the evaluation of home-visiting nurses. We analyzed the results of a questionnaire survey of visiting nurses working at 177 visiting nursing stations in A prefecture. Questionnaire forms were sent to 531 nurses, and 208 responses were collected (response rate: 39.2%). On a 10-level scale of “satisfaction with home-visit nurse discharge support”, 119 nurses (60.4%) were in the low group with scores of 1 to 5, and 78 nurses (39.6%) were in the high group with scores of 6 to 10.Logistic regression analysis identified four items as factors related to satisfaction with discharge support:“Information collection on post-discharge life”(OR: 5.861),“Regional cooperation with hospitals”(OR: 5.185),“Experience receiving requests and complaints for discharge support from service users”(OR: 2.351), and“Visiting nurse experience”(OR: 1.004). Regarding the specific content of discharge support, we found that support for treatment and illness was provided; however, support for living environment and nursing care was insufficient. Our findings suggest that the quality of discharge support will improve if ward nurses are aware of such support and become involved with patients. |
