本研究の目的は，長期隔離処遇患者の行動制限最小化に向けた多職種間における看護師の役割を明らかにすることである．精神科医療従事者11人を対象に半構造化面接を行い，質的記述的研究の手法によりデータを分析した．分析の結果，関係構築の段階では【時間を共有し回復の糸口を探す】【困りごとの解決を積み重ねる】，行動制限緩和に向けた目標設定の段階では【開放観察の方策を検討する】【多職種の視点を取り入れながら試行錯誤し方策を立てる】，行動制限緩和のための具体的方策の段階では【療養環境を整える】【職種の特性を活かしてかかわる】，変化を評価して次の実践につなげる段階では【精神状態の小さな変化に気づく】【経験を共有し次の実践につなげる】のカテゴリが明らかとなった．看護師は多職種連携の際に，各職種の特性を発揮できるようにマネジメントし，開放観察の方策や行動制限緩和のタイミングを伝える役割を担うことが示された． |

The purpose of this study is to clarify the roles played by nurses in a multidisciplinary setting for minimizing behavioral restrictions in patients undergoing long-term isolation. Semi-structured interviews were conducted with 11 psychiatric medical professionals, and a qualitative descriptive analysis was performed. As a result of the analysis, the categories “sharing time and looking for clues to recovery” and “accumulating solutions for each problem” were clarified in the relationship-building stage. The categories “examining measures for open observation” and “establishing methods through trial and error while incorporating multidisciplinary perspectives” were clarified at the stage in which targets are set for reducing activity restrictions. The categories “preparing a therapeutic environment” and “taking advantage of the characteristics of the occupation” were clarified at the stage in which concrete measures are taken to ease behavior restrictions. The categories “noticing small changes in the mental state” and “sharing experiences and connecting them to the next practice” were clarified at the stage in which change is evaluated and connected to the next practice. This study showed that nurses manage multidisciplinary collaboration in a way that allows the characteristics of each occupation to be demonstrated and play a role in disseminating measures for open observation and the timing of reducing activity restrictions. |
