| 本研究では患者本人による自己評価と同伴家族による他者評価の違いをDASC-21 （Dementia Assessment Sheet for Community-based Integrated Care System 21items）の質問項目を用いて鑑別診断別に明らかにし，診断後支援の手がかりを検討することを目的とした．対象者は，2016年4月～2017年3月までに都道府県指定の認知症疾患医療センター専門外来を家族同伴で初診した48名（男性25名，女性23名，平均年齢76.02±6.89歳）であった．評価項目は，認知機能検査およびDASC-21による自己評価と他者評価の点数，鑑別診断名および検査時や診察時に語られた会話内容を用いた．対象者を鑑別診断名により，正常範囲内群・軽度認知障害群・アルツハイマー型認知症群の3群に分け，DASC-21の総得点および下位7カテゴリの得点別に自己評価と他者評価との得点差から3群の特徴をつかんだ．結果，正常範囲群でも評価に差がある症例があり，軽度認知障害群，アルツハイマー型認知症群へと病のステージが進むにつれて自己評価よりも他者評価が低いと評価された症例数も下位カテゴリ数も増えていた．自己評価が他者評価よりも低い症例では，患者本人への心理的支援が，他者評価が自己評価より低い症例では，家族に対する心理教育の実施が有益だと思われた．認知機能障害や生活障害に対する本人と家族の評価や視点の違いを，DASC-21の項目を用いて評価することにより，診断後支援につながるまでの「空白の期間」の短縮に貢献する可能性がある． | ||||||||||

| The purpose of this study was to clarify differences between patients’ self-evaluations and family members’ evaluations of others by differential diagnosis using the DASC-21 questionnaire, and to examine clues for postdiagnostic support. The subjects included 48 patients (25 males and 23 females, mean age 76.02±6.89 years) who had first visited a specialized outpatient clinic accompanied by family members between April 2016 and March 2017. The evaluation items were cognitive function tests, scores of self-assessments and others’ assessment by DASC-21, differential diagnosis, and the content of conversation during the examination and consultation. Subjects were divided into three groups by differential diagnosis: within normal limits, mild cognitive impairment, and Alzheimer’s disease groups. Those who were rated lower by others than by their self-evaluation increased in frequency as the disease stage progressed, as did the number of subcategories. Thus, patients with low self-evaluations may benefit from psychological support, and those with low family member evaluations may benefit from psychological family member education. Patient and family assessment using the DASC-21 questionnaire may contribute to reducing time from diagnosis to support. | ||||||||||
