| 本邦の透析患者数は増加の一途を辿り，その半数以上が65歳以上の高齢者である．超高齢社会の社会保障制度を維持するため，現在の病院中心の医療から地域で完結できる在宅医療を基盤に置いた医療への転換の必要性が叫ばれているが，9割以上の透析患者は施設透析を選択しており在宅透析はわずかである．そこで，在宅透析の普及策としてassisted PD（介助者の支援による腹膜透析：assisted peritoneal dialysis）の活用が注目されている．assisted PD の普及には社会資源の利活用が重要であり，介護施設での腹膜透析患者の受け入れ状況に対して介護報酬の設定等の改正が必要である．また，専門医など専門知識のある職員が在籍する導入・管理病院が，地域のクリニックや施設に急変時の対応の仕方や感染対策などの教育的な研修を行い，患者が地域へ帰っても安定した透析管理が行うことができるような支援の充実が望まれる．さらに，急変時や退院などで施設間の移動がある際や多職種で高齢腹膜透析患者の治療にあたる際に，円滑な情報共有がしやすいようにICT を活用した医療機関・介護施設・家庭が相互の利用できるネットワークを構築することが必要である． | ||||||||||

| The number of dialysis patients in Japan continues to increase, and more than half of them are aged 65 years or older. In order to maintain the social security system in a super-aged society, there is a need to shift from the current hospital-based medical care to home-based medical care that can be completed in the community. Therefore, the use of assisted peritoneal dialysis (assisted PD) has been attracting attention as a measure to promote home dialysis, and it is important to utilize social resources to spread assisted PD. Revision of the nursing care reimbursement system is necessary for the acceptance of patients in nursing care facilities. In addition, it is desirable for hospitals with specialist doctors and other knowledgeable staff to provide local clinics and facilities with educational training on how to respond to sudden changes and infection control measures, and to enhance support for stable dialysis management even after patients return to the community. Furthermore, it is necessary to establish a network that can be used mutually by medical institutions, nursing care facilities, and homes, utilizing ICT, to facilitate smooth information sharing when patients are transferred between facilities due to sudden changes or discharges, or when multiple professions treat elderly peritoneal dialysis patients. | ||||||||||
