| 本研究は未就学の医療的ケアを要する児童及びその家族が在宅生活を送るための支援及び支援体制を検討したものである．地域において医療的ケア児やその家族を支える多職種支援体制の整備はニー ズが高いが，医療的ケア児やその家族が受けることのできるサービスは地域ごとに実状が異なり，医療のみならず教育，福祉，子育て支援，保健等の多岐にわたる．また窓口も様々あるため支援の輪の中心であるべきサービスの受け手がその現状を把握することは困難である．そのため，各専門分野が有機的に連携・協力することが必要であり，医療的ケア児等コーディネーターを中心としたチームケアを展開する中で地域の人的資源及び物的資源の体制を整備していくことで，医療的ケア児の発達保障に繋がると考える．また今後の課題として，当事者を支援機関につなぐ相談ルート・ニーズや具体的な人的資源・物的資源などの支援体制をどのように整備していくかを明らかにする必要がある． | ||||||||||

| This study examined support and support systems for preschool children and their families who require medical care to live at home．There is a great need to develop a multidisciplinary support system to support children with medical care and their families in the community, but the services that children with medical care and their families can receive differ from region to region, and not only medical care but also education, welfare, child-rearing support, and health care etc. In addition, since there are various contact points, it is very difficult for the recipient of the service, who should be the center of the support network, to grasp the current situation. Therefore, it is necessary for each specialized field to collaborate and cooperate organically, and while developing team care centered on coordinators such as for children with medical care, we have established a system of human resources and material resources. We believe this will lead to the developmental security of children with medical care. In addition, as a future issue, it is necessary to clarity how to develop a support system that includes consultation routes, needs, and specific human and material resources to connect the concerned parties to support organizations. | ||||||||||
