| 膵臓がんは予後が悪く長期生存が困難である為，短い時間の中で病状を受けとめながら，折にふれて治療方針の決定を迫られたり，療養の場の変更を余儀なくされたりといったことも少なくはない． 本研究では，文献から読み取ることができる膵臓がん患者の意思決定に係る患者と家族の体験を整理し，今後の課題を検討した．これによって，患者・家族が終末期にむけて見通しを立てていることと， 支援する側の見通しに違いが生じ，話し合うタイミングを逃してしまうことが懸念されることが明らかとなった．今後，療養生活を継続する期間に現れる意思決定の課題を具体的にするとともに，その時を見据えて治療・緩和ケア・療養について十分に話し合え，患者の意思決定支援の方法や内容を検 討することが必要であると考える． | ||||||||||

| Convalescence during treatment for pancreatic cancer is difficult and the prognosis for a long life is poor. A patient can develop pancreatic cancer over a short period of time. Further challenges may include difficulty in making decisions about treatment or having to change the location of medical treatment. In this study, we summarized the experiences of patients and their families related to decision-making of pancreatic cancer patients that can be read from the literature and examined future issues. This study revealed that there is a difference between the patient’s and family’s outlook toward the end of life and that of the caregivers, and that there is a concern that the timing of discussion may be missed. In the future, it is necessary to clarify the issues of decision-making that will appear during the period of life sustaining treatment, and also to consider methods and contents that allow sufficient discussion of treatment, palliative care, and recuperation in anticipation of that time. | ||||||||||
