| 本稿では，在宅医療の制度上の位置付けを明らかにした上で，施設基準を参考に，我が国の在宅療 養支援診療所と機能強化型在宅療養支援診療所の定義を確認した．次に，岡山県の倉敷市・総社市の在宅医療の実践のうち，機能強化型在宅療養支援診療所の経営実践の特徴として，（1）有床診療所であり，医療法人だけでなく社会福祉法人を有し，2法人を合わせて13事業所を有している法人，（2）訪問診療専門のクリニック，（3）小児在宅部門を有しているクリニック，（4）院長が他の在宅療養支援診療所に在籍していた総社市の2つのクリニックの存在，（5）複数の医療・介護サービスや地域医療連携室を有するクリニックの存在，の5点を示した。また，そうした5点の特徴から，（1）有床診療所のメリット，（2）複数の事業所の1つとしての在宅療養診療所がどのように機能しているか，（3）小児在宅診療部門はどのように機能しているか，（4）管理栄養士の役割や在宅患者の褥瘡対策の事例研究，（5）もの忘れ認知症外来の地域で果たす役割や，1クリニック内の外来診療と在宅診療が，実際の患者からすればどのように機能しているか，の5つを今後の課題を整理した。 | ||||||||

| In this paper, after clarifying the systematic position of home medical care, we confirmed the definitions of Japan’s home medical care support clinics and function-enhanced home medical care support clinics with reference to facility standards. Next, of the practice of home medical care in Kurashiki City and Soja City in Okayama Prefecture, we indicated that, as a characteristic of the management practice of the function-enhanced home medical care support clinic, (1) there are 13 establishments of bed-based clinics, which are not only medical corporations, but also social welfare corporations, (2) a clinic specializing in visiting medical care, (3) a clinic with a pediatric home-based department, (4) two clinics in Soja city where the director of one is enrolled in the other (5), and there are five clinics that have multiple medical/nursing services and regional medical cooperation rooms．Based on these five characteristics, we have arranged five future issues regarding how they function from the viewpoint of actual patients: (1) the merits of a bed-based clinics, (2) how a home-based medical clinic as one of multiple establishments works, (3) how the pediatric home-based medical department works, (4) the role of a registered dietitian and case study of pressure ulcer measures for home patients, and (5) the role to play in the community of forgetfulness dementia outpatients, and outpatient and home care in one clinic. | ||||||||
