A大学の看護学生3年生を対象に，老年看護学実習の学内演習で身体拘束のシミュレーション演習を行った．本研究の目的は，看護基礎教育における倫理教育への示唆を得るために，身体拘束のシミュ レーション演習において，学生が捉えた高齢者の思いを明らかにすることである．身体拘束のシミュレーション演習後の，学生レポートの「身体拘束された高齢者の思い」に記載のあった内容を質的統 合法（KJ 法）で分析した．レポートは評価が終わった前年度のものを使用した．110名中，87名から同意が得られ，その中で高齢者役をした55名のレポートが対象となった．323のラベルが7段階で最終6つに統合された．学生は，拘束される高齢者の底知れない苦しみが，拘束自体による苦痛と，看護師に理解されない状況から，ますます深まっていく体験をしていた．さらに，安全を目的としているにもかかわらず，拘束中に動くことは余計に危険であり，拘束中の看護と解除を考え続けてほしいと考えていた．つまり，看護の責任についても感じていた．本演習により学生は，拘束の身体的，精神的，スピリチュアルな側面の弊害が理解でき，それらに対する配慮が必要であること，看護の責任に まで考えをめぐらすことができていた．本演習は看護学生の倫理的感受性を育む一助となった可能性がある．今後は，拘束縮小につながる具体的な対策を知ることと，倫理的問題に対処する問題解決能 力を身に付けることが課題である． |

This study examined nursing students’experience in a physical restraint simulation, focusing on the emotions of restrained elderly patients. One-hundred and ten nursing students submitted reports entitled“Restrained Elderly Patients’Emotions”after a physical restraint simulation. Eighty-seven agreed to analyze their reports for our study. We analyzed the reports of fifty-five students who played the role of the elderly by an integrative and qualitative (KJ) method. Students under restraint experienced physical distress caused by the restraint and emotional distress due to nurses’insufficient understanding. With both types of distress intensifying, the students realized the possibility of physical restraint leading to dangerous situations. They also considered the responsibility of nurses who perform it. The physical restraint simulation promoted students’understanding of the negative physical, mental, and spiritual effects of physical restraint, supporting the usefulness of such a simulation to help students develop ethical sensitivity. Problem-solving skill acquisition may be the next challenge for them. |
