| 顎裂部骨移植術後の離床場面では，腸骨を削ったことによる強い痛みを抱えている患児と，患児を心配しながら見守る保護者との両者の状況や反応を捉えながら，看護師は離床を進める必要がある． 本研究は，顎裂部骨移植術を受けた患児の離床時における看護援助の力と技を明らかにすることを目 的に，エスノグラフィーの方法を用いて，患児，保護者，看護師の3者を1組とする計4組を対象とし， 参加観察と半構成的面接を行った．その結果，看護師の援助として（1）離床前に痛みの程度から離床可能を判断し，恐怖を取り除きながら離床継続に努める（2）腸骨部への負担軽減のため看護師主体でベッドアップから一気に端座位まで進める（3）子どものがんばりを認め，自信に変えて離床を進める（4）我が子を気づかう保護者の思いを大切にしながら離床を進める（5）がんばって離床した後の子どもに鎮痛剤の使用を提案するの5つのテーマが明らかになった．看護師は，顎裂部骨移植術後の子どもの苦痛を理解した上で離床の可否を判断する力，痛みと恐怖を落ち着かせて子どもの主体性を引き出しながら離床を進める技，保護者の思いの尊重と専門職としての役割発揮のバランスを調整する力を持ち備えておく必要があると示唆された． | ||||||||||

| With the aim of investigating nursing assistance provided when initiating out-of-bed mobilization of pediatric patients who undergone alveolar bone grafting, participant observation and a semi-structured interview were conducted in 4 groups consisting of 3 subjects (pediatric patient, parent, and nurse) in each group, and the obtained data were analyzed using ethnography. As the results, the following 5 themes were identified as nursing assistance: (1) Before the patient gets out of bed, judge whether the patient can be mobilized out of bed based on the degree of pain, and keep encouraging mobilization of the patient by reducing their anxiety, (2) To reduce the burden on the iliac region, elevate the patient’s torso and move the patient into a sitting position on the edge of the bed, (3) Promote mobilization of the patient by helping him/her be proud of what he/she has accomplished, (4) Promote mobilization of the patient while considering the feelings of parents who care for their children, and (5) Propose the use of analgesics in children for their mobilization out of bed. The results indicated that the nurses regarded mobilization as the best benefit of the children, based on which they evaluated the pain, brought out the strength of children, and provided support to parents while paying attention to their feelings. |
