| 医療機関が実効性のある経営戦略を策定するためには，政府等の公開データを活用して地域医療の 状況を推定する必要がある．近年，NDBオープンデータと病床機能報告データが公開されたことで， 従来の研究では不可能であった視点から，地域医療の状況の推定が可能になったと考えられる．本研究の目的は NDBオープンデータと病床機能報告データを用いることで，地域医療の状況をどの程度推定できるか，また，それらのデータの活用に係る課題も明らかにすることである．本研究では岡山県の入院医療における手術実施状況を推定した事例を示した．NDBオープンデータと国勢調査データを用いて推計した各市町村の部位別手術発生数と，病床機能報告データから算出した各市町村の部位別手術実施数を比較し，各市町村と各二次医療圏の部位別手術実施状況を推定した．その結果，入院医療における手術実施状況は二次医療圏単位での評価が適切であると考えられ，岡山県では「県南東部」が中心的な役割を担っており，他の二次医療圏から多くの患者が流入していることが示唆された．それは第8次岡山県保健医療計画の内容と概ね一致する結果であった．本研究により，NDBオープンデータと病床機能報告データを用いることで，岡山県の入院医療における手術実施状況を部位別に推定可能であることが示された．NDBオープンデータと病床機能報告データには改善の余地が残されており，NDBオープンデータは各二次医療圏の各分類名称の発生数，病床機能報告データは年間の各医療機関の部位別手術実施数を公開することが望まれる． | ||||||||

| In order for medical institutions to formulate effective management strategies, it is necessary to estimate the situation of regional healthcare using the available public data of the governments. The purpose of this study is to clarify to what extent the situation of regional healthcare can be estimated by using NDB open data and sickbed function reports data, and also clarify the issues related to the utilization of these data. In this study, we showed an example of estimating the implementation status of surgery in inpatient care in Okayama Prefecture. First, we estimated the number of surgeries by organ that occurred in each region using NDB open data and the census data, and calculated the number of surgical cases for each organ performed in each region from sickbed function reports data. Then, we compared the number of surgeries that occurred in each region with the number of surgical cases performed, and estimated the implementation status of surgery for each organ in each region. As a result, it was suggested that the“Southeastern Part of the Prefecture”plays a central role in Okayama Prefecture, and many patients are flowing into the“Southeastern Part of the Prefecture”from other regions. This study has shown that it is possible to estimate the implementation status of surgery by organ in inpatient care in Okayama Prefecture by using NDB open data and sickbed function reports data. NDB open data and sickbed function reports data have room for improvement. It is considered necessary to improve versatility by standardizing the granularity of data in anticipation of secondary use of public data. | ||||||||
