| がん患者はがん罹患に伴って身体的にも社会的にもさまざまな変化に直面し，生活のあらゆる面を 整えていく必要に迫られる．がん患者が身体面，生活面を調整していく力を獲得，強化することは， がん患者がさまざまな困難を克服しがん罹患後の生活に適応していくために重要だといえる．通院し ながら生活するがん患者が，身体や生活に密接した課題をひとつひとつ調整して自分らしさや安定し た状態を獲得していくために必要な患者の調整力と関連する事柄を明らかにする．初期治療後の退院 から6ヶ月以上経過し，経過観察もしくは継続治療目的で外来に通院中のがん患者13名を対象とし， 半構成的面接を行った．面接によって得たデータはケース毎に逐語録を作成し，がん罹患後の生活に おいて成果を目指した認知的・行動的な努力が語られている内容を抽出，コード化した．コード化し たものをさらに類似したものでまとめてカテゴリー化し，調整力の構成要素，調整力に関連する事柄 として分類した．調整力は自分の状況を踏まえた上で具体的な調整の方法を【考案する力】，考えた 調整方法や必要なことを【実行する力】，調整の結果を振り返り，意義や有効性を【評価する力】，判 断基準となる【拠り所をつかむ力】，価値観や気持ちの【方向性を変える力】，調整方法に幅をもたせ ていくために【調整方法を拡大する力】から構成されていることがわかった．調整力に関連する事柄 として，【サポーターの存在】，【身体症状】，【役割認識】の3つが抽出された．通院しながら生活する がん患者の調整力の特徴として，身体の微細な変化を捉えて調整方法を探ること，状況に応じて調整 方法を変えること，活動に焦点を当てて調整をしていることが考えられた． | ||||||||

| To aim was to clarify the adjustment ability and related factors of outpatients living with cancer. Semi-structured interviews following hospital discharge were undertaken over a period of at least six months with 13 outpatients who had cancer in order to observe their developments or continued treatment. The data obtained from the interviews were transcribed verbatim for each case, and the codified data were categorized and classified into elements comprising adjustment ability and related factors. Adjustment ability comprised the [ability to devise] specific strategies of adjustment based on one’s situation, the [ability to implement] devised adjustment strategies and what was deemed necessary, the [ability to evaluate] significance and effectiveness by reflecting on the adjustment results, the [ability to gain a foundation] for criteria, the [ability to change the orientation] of values and feelings, and the [ability to broaden coping strategies] to allow for latitude in them. [Existence of a supporter], [physical symptoms], and [role perception] were identified as factors relating to adjustment ability. Exploring coping strategies for minute changes in the body and making adjustments focusing on activities were considered the adjustment abilities of outpatients living with cancer. | ||||||||
