| 本研究の目的は, 国内と海外のレビューを通してLower Urinary Tract Symptoms :LUTS の実態とそれに対する支援について整理し,地域在住高齢者の排尿に関連した生活の困りごとに対する支援 システム構築への示唆を得ることであった.医学中央雑誌,CINAHL および MEDLINE を用いて, 国内文献は,「尿失禁」あるいは「夜間頻尿」,あるいは「排尿困難」あるいは「過活動膀胱」と「高 齢者」と「地域」あるいは「支援」のキーワードで文献検索を行った結果39件が抽出された.海外文 献は,"lower urinary tract symptoms"と"elderly"と"epidemiology"と"community"あるい は"difficult"あるいは"nurse"あるいは"support"のキーワードで文献検索を行った結果,137件 が抽出された.文献内容を検討した結果,以下の内容が明らかになった.1.地域在住高齢者におけ る LUTS については尿失禁に焦点を当てて調査されたものが大半であった.2.国内の地域在住高齢 者における LUTS の生活への影響については,詳細な部分まで明確になっていないことが推察された.3.地域在住高齢者の LUTS に対する支援は少なく,尿失禁の改善を期待した運動プログラムやそれ を継続していくためのものであった.4.高齢者の排尿に関するニーズや地域特性をふまえた支援シ ステムと,その評価方法の検討の必要性が示された. | ||||||||||

| The purpose of this study was to gather information on the current conditions and the support available for Lower Urinary Tract Symptoms (or LUTS) by means of a national and international literature review and to get suggestions for the construction of a support system for elderly persons living in regional communities with uncertainties associated with urination. Literature in Japan was searched by using the keywords"elderly," "community,""support,""urinary incontinence,""nocturia,""unrinary difficulties"or"overactive bladder"in Igaku Chuuou Zasshi. 39 items were extracted. Overseas literature was searched by using the keywords of"lower urinary tract symptoms,""elderly,""epidemiology,""community,""difficult,""nurse"or"support"in CINAHL and MEDLINE. 137 items were extracted.The results of the document retrieval performed indicated the following.(1) The majority of the literature concerning LUTS among the elderly living in regional communities involved surveys focused on urinary incontinence.(2) The lack of a detailed understanding of the influence of LUTS on the lives of the elderly in regional communities nationally was inferred.(3) Support for the elderly in regional communities with LUTS was low, and consists of exercise programs to alleviate incontinence and the efforts to continue the programs.(4) The results showed the need for support systems based on the needs of elderly persons with urination problems in light of the regional peculiarities and the methods to make those assessments. | ||||||||||
