我々は,医療現場で電子カルテシステムが導入された場合に,これを支援する人材,主に診療情報管理士に必要と考えられる知識・技能を,演習を通じて習得することを目的とした「電子カルテ・ラボ」学習支援システムを開発した.本システムは,電子カルテシステムの基本操作を中心とした,技能の習得を目的とした機能や,知識の習得を目的としたe-Learning機能があり,テストによる自己学習を通じて理解が深められる設計となっている.そのテストにより学習効果をより高めるため,テスト項目の評価を行った.テストは30項目からなる,多肢選択テストである.分析には,38名の学生を対象に実施したデータを用いた.評価は,各項目の正答率,識別指数を用いて,テストの内容的妥当性を検討した.項目の正答率が0.2以上,0.8以下,識別指数が0.2以上を望ましい項目とすると,30項目中,適合項目は12項目(40%)であった.適合項目に関して自信度と正答率・識別指数の関係を調べたところ,どちらも中程度の相関が見られた.テスト項目を評価する際に,自信度を調査することは,有用であると考えられる.最後に,項目反応理論を使用して分析を行った.正答率と識別指数を用いて行った項目の分類と項目特性曲線とは良く対応し,テストの評価方法として一定の有用性が認められた. |

We developed an educational system called the "EPR (Electronic Patient Record) Laboratory". The system was implemented in a course entitled "Health Information Manager Practice" which forms part of the Department of Health Informatics curriculum. The EPR Laboratory is designed for students to acquire knowledge and skills of EPR and EPR systems. The system incorporates fundamental functionalities of EPR systems and e-Learning capabilities such as self-assessment tests. To improve the quality of the tests, we conducted an evaluative study of the test items. As a preparatory study, we gave a test consisting of 30 multiple-choice questions, and analyzed the results of 38 students. The correct answer rate and discriminatory index were calculated. The criteria for suitable questions consisted in the correct answer rate being between 0.2 and 0.8, and the discriminatory index being over 0.2. Given these criteria 12 out of the 30 questions (40%)-fitted the criteria. As part of the evaluative test, we asked the students about their confidence in answering each question. As a result of the analysis, the confidence level was considered useful in evaluating the quality of the tests. Lastly we applied Item Response Theory to the test data. There was discernable correspondence between the results of "discriminatory index and correct answer rate" and the Item Response Curve. |
