| 本研究の目的は,社会構築主義の立場から健康や福祉の目標を「生きている」ことの充実感そのもの (=健康自尊意識 HE:Health Esteem) として捉え,その境地にあるハンセン病当事者の語りを通して,健康自尊意識 (HE) の規定要因を,対話的構築主義アプローチの手法を用い構造分析をなすことである.これに関してすでに本学会誌上にて,「医療福祉学に基づく健康格差に関する研究 (2) -ハンセン病問題当事者のライフストーリーにみる健康自尊意識(HE)」研究を試みたところである (熊谷他,2009).本稿では,その考察結果の普遍性を追究すべく,別のハンセン病当事者の聞き取りを通して,同じく対話的構築主義アプローチを用い構造分析を試みた.抽出されたコードは「12歳での入所」「退所そして再入所」「赤痢の罹患そして病気の進行」「何回死のうとしたかわからない時代」「新薬プロミンの登場」「アプレゲールの時代」「救いを友人と宗教に求めた時代」「患者運動の中で」「自分を表現する手段としての点字」「ハーモニカ楽団の結成」「ミッションの招きでスイスへ」の11項目に及んだ.ライフストーリーの構造分析の結果、先行研究ですでに検証されている「マスターナラティヴ」「モデルストーリー」さらに「新しいストーリー」の抽出とその力動性が再確認された.また,健康自尊意識 (HE) の形成要因についても「利用者文化に支えられたストレングス要因」と「実体ある解放・復権の要因」が検証され,加えて「公からの他者承認の要因」が健康自尊意識 (HE) に影響を及ぼしていることが明らかになった. | ||||||||||

| The purpose of this study is to examine set factors in Health Esteem (HE) which were discussed previously in "A Study of Inequalities in Health based on Medical Welfare (2) -Health Esteem (HE) in the Life Story of a Hansen's Patient-" through the method of a`life story study', and in particular, through the interactive construction approach. We interviewed another (not the former) Hansen's patient for his life story, and conducted a structural analysis of it according to the procedure we set forth (a procedure consisting of three parts : writing down the story, extracting the codes and interpretation). We were able to find 11 codes from the life story of Mr. Takahashi. As a result of the structural analysis of his life story, our study re-verified `the master narrative', `the model story' and 'the new story' which we presented in our study based on Sakurai's study. Our structural analysis also reconfirmed that the factors of the strength of living power and `substantial restitution' are related to Health Esteem. In addition, our study gained the new insight that the factor of other people's (social) approval (`OK') is also related to it. | ||||||||||
