| 社会の変化と共に乳児院の役割は,孤児のケアから複雑多様な問題をもつ親子へのケアに変化してきた.そのような変化に伴う親子のケアを実践していくためには看護師および保育士の業務の補完が必要とされる.しかし,現状は保育士に比べ看護師の就業数が少なく,保育士による業務の補完が大半を占め,看護師の専門性が脆弱な状態にある.本研究は,乳児院での保育看護実践において看護師の専門性を活かした看護ニーズを検討するために,全国の乳児院に就業している看護師および保育士に質問紙調査を行った.調査内容は,現在の保育看護業務の実施状況,職員の専門性発揮の状況などである.その結果,保育看護実践には子どもの健全な成長発達を促すための日常生活に関する援助と家族再統合に向けた子育て支援に関する援助に大別できた.これらの業務は,看護師と保育士の専門性を活かした協働体制が必要であることが明らかとなった.また乳児院への看護師就業者が少ない現状は,基礎教育において乳児院に看護師が就業していることを学ぶ機会がほとんど得られていないことに起因していると考えられる.看護師は長期間の臨地実習で患者の問題点に対する計画立案・実践・評価を習得しており,自立支援計画策定や保育計画などを担うことができ,保育看護実践において必要な専門職である.そのためにも,看護師確保に向けて基礎看護教育で乳児院における看護師の役割について啓発することは重要な課題である. | ||||||||||

| The role of the infant home has changed from caring for orphaned children to caring for the diverse factors in parental relations. Nurses and other childcare personnel share in providing for the care and needs of the children. However, there is an insufficiency of nurses now in infant homes. As a result, the specialties in nursing in this area are weakening. The purpose of this study is to examine the nursing needs of the childcare nursing practices, with an emphasis on the specialties of the nurses who work in infant homes. The method used was a questionnaire survey given to childcare personnel along with the nurses who work in infant homes across Japan. Our results show that childcare nursing practices involve care for everyday life and seeking ways to promote the healthy development of children. They also aim at providing a childcare support system for family reunification. These duties require a collaborative system in order to make good use of the specialties of nurses and childcare personnel. There are fewer opportunities for nurses to learn the special skills needed to work effectively in infant homes. This is a major reason behind nurses not desiring to work in infant homes. The important task for the future is to enlighten nursing students about the role of nurses working in infant homes. | ||||||||||
