本稿の目的は,医療福祉学に基づく健康格差に関する研究(1)で提起された健康自尊意識(HE:Health Esteem)の規定要因を,ライフストーリー研究法,とりわけ対話的構築主義アプローチの手法により考察することにある.ハンセン病問題当事者の聞き取りを行い,桜井が示した手順(書き起こし手順,コードの抽出,解釈)に従いライフストーリーの構造分析を試みた.コードの抽出は「病者にとっての療養所の意味」「社会情勢と療養所に入ること」「病気の判明と職場の解雇」「はじめての大学病院受診」「療養所までの道のり」「回春寮の消毒風呂」「療養所での生きる決心」「離れていく家族との距離」「プロミンの登場も誰も治ったとは思えなかった」「家族以上に大きい同病者のつながり」「予防法廃止・国の訴訟断念の意味」の11項目に及んだ.ライフストーリーの構造分析の結果,先行研究者である桜井の提起した「マスター・ナラティブ」「モデル・ストーリー」さらに「新しいストーリー」が検証された.ただし桜井が提起した「モデル・ストーリー」は外にむけられることに対し,本研究では内にむけられるものを含むことに新しい知見が得られたと考える.また,このライフストーリーの構造分析により,個別のライフストーリーから抽出されるストーリーの歴史性,力動性の要因がHE(健康自尊意識)に影響をおよぼしていることが検証されたといえる. |

The purpose of this study is to examine set factors in Health Esteem (HE) which were discussed previously in `A Study on Inequalities in Health based on Medical Welfare (1)' through the method of a `life story study,' in particular, through the interactive construction approach. We interviewed a Hansen's patient for his life story, and conducted a structural analysis of it according to the procedure Sakurai set forth (a procedure consisting of three parts: writing down the story, extracting the codes and interpretation). We were able to find 11 codes from the life story of Mr. Hiroi. As a result of the structural analysis of his life story, our study verified the `master narrative,' `the model story' and `the new story' which Sakurai presented in his research. In addition, our study gained the new insight that `the model story' is not only directed `outwardly' as Sakurai asserted, but directed `inwardly' as well. Our structural analysis also demonstrated that the factors of the historicalness and the force dynamics of an individual life story are related to Health Esteem (HE). |
