本稿の目的は,特に,近年,社会資源の数のみから健康格差を捉える状況にアンチテーゼを唱え,「健康格差」が,健康増進にかかわる資源の実現可能性(利用可能性)と健康増進の資源に対する重みづけ(重要と考えている度合い)の格差であることを明らかにすることである.その中で,Health Esteem(健康自尊意識)という新たな概念を提唱したい.本稿では,健康格差を「本人の自己責任を超えた,社会的要因によって生じた属性に起因する,社会的に許容される一定の範域を超えた健康に関する差である」とし,医療福祉学の視点から健康の定式化を試みる.ただし,社会調査の性質上,医学的な健康状態,客観的重みづけが測定できないため,健康状態を定量的に扱うことは非常に困難である.しかしながら,本稿では,健康状態の測定そのものよりも,その定式の中で用いた実現可能性や重みづけが健康格差の重要な要素であると考える.本稿では,ある要素に対する重みづけが高いということは,HE(Health Esteem)という健康自尊意識が高いと考えた.HEとは,自身の健康を有意味であると捉えることができる意識である.HEは同じく個人の責任を超えた構造の影響を受けると同時に,個人の資質にも影響される.医療福祉学の視点から健康格差を考えるとき,まず我々はHEのどの側面が格差であり,また個人の資質の問題なのかを精査する必要がある. |

The purpose of this article is to insist on an antithesis against the ongoing situation where the term "inequalities in health" is used too lightly and to make clear that "inequalities in health" is the difference of the availability of the resources related to health improvement and the weight placed on the importance of the resource. We would like to advocate a new concept "Health Esteem (HE)" for such a situation. In this article we define inequalities in health as "difference related to health beyond a certain category socially accepted and attributed to the qualities of social properties other than a person's self-responsibility" and try to formulate health from the viewpoint of medical welfare. However, it is very difficult to handle health situation quantitatively because of a lack of measurement of medical health status and objective weight owing to the nature of social research. Here we regard availability and weight used in this formulation as important elements of inequalities in health rather than measurement of health status. We consider that high weight on resources means high HE. HE means sense to perceive one's health as important. HE is affected by qualities of the individual as well as structures beyond his/her own responsibility. |
