| 本研究は,うつ病患者に面接調査し,従来の研究にはなかった患者の観点からうつ病患者の回復体験を捉える記述研究である.うつ病患者が入院・療養し退院するまでの期間において改善したと認識した状態について,Grounded Theory Approachを用いて,12名のうつ病患者の回復プロセスを明らかにした.対象者は,男性4名,女性8名で,平均年齢51.6歳(範囲28±72歳)であった.うつ病患者の回復過程において,発病により枯渇した《エネルギーが充足》していく時に,患者は改善したと認識していた.《エネルギーの充足》プロセスには,ネガティブ思考からの〔開き直り〕,〔治りたい〕意志の出現,試行錯誤して行動する〔案ずるより産むが易し〕,症状の改善に伴う〔生活の拡大〕,生き生きとした〔感情と感覚の蘇り〕の5つの段階が存在した.この段階を促進していく要素として,〔抗うつ薬の効果〕,〔症状の改善〕,〔主体性の回復〕の3つが関連していた.これらはうつ病の代表的な徴候および症状であり,医療者が治療の効果を観察する項目である.以上のことより,うつ病患者の看護においては,従来観察していたうつ病の症状や徴候を観察するだけでなく,患者自身が自己の改善を今どのように捉えているかを把握することが重要であることが判明した.また,療養過程の初期では,意欲や思考障害の状態に重点をおき,後半では,行動面に重点をおいた観察と働きかけに意義があることが示唆された. | ||||||||

| The self-recognition of improvement in depression patients from the time of admission in the hospital and recuperation to after discharge from the hospital was assessed and clarified by the Grounded Theory Approach. Subjects were 12 patients (4 male and 8 female, mean age was 51.6; range 28-72 years old). The result was that patients recognized improvement of the `restoration of energy', which had been depleted by the disease. In the `restoration of energy' process, the author identified 5 ascending stages : the `so-what' attitude, showing the will to get better, the `don't think just do' attitude, the broadening of everyday life and the restoration of feeling and senses. The author found that three further factors related to facilitating these stages : the effect of antidepressants, the improvement of symptoms and the revival of independence. These factors were representative manifestations and symptoms of depression, as well as the tools that healthcare providers use for the assessment of treatment effectiveness. The author believes these findings will be of significant benefit to those involved in nursing depression patients. | ||||||||
