|
目的は在宅高齢者の健康を構成する因子の影響関係を定量的に明らかにし,さらに健康関連因子と主観的健康感との関連を明らかにすることである. 方法は全国11市町村の入院,入所を除く65歳以上在宅高齢者27,472人を調査対象に郵送法により質問紙調査を行った.調査回答者は21,716人(回答率79.0%),分析対象者は19,416人であった. 因子分析で4つの潜在変数を抽出し,国際生活機能分類 の定義に準拠して「心身機能(一病息災的健康)」(以下「」は潜在変数を示す),「活動(生活能力)」,「社会参加」および「社会支援」と命名した健康構造モデルを作成し,影響の方向とその強さを求めた.分析は共分散構造分析を用いた. 結果:潜在因子間の影響関係は次のようであった(いずれもパス係数は有意). 1.「心身機能(一病息災的健康)」は,「活動(生活能力)」から直接影響を受け,「社会参加」からは「活動(生活能力)」を経由して間接的に影響. 2.「社会支援」は「心身機能(一病息災的健康)」に「社会参加」,「活動(生活能力)」を経由して間接的に影響しており,直接影響は弱い. 3.観測変数である主観的健康感は,性や年齢に関係なく,「心身機能(一病息災的健康)」に最も強く関連. 【結論】 健康構成要因の影響関係は「社会参加」をすることで「活動(生活能力)」が活性化し,「心身機能(一病息災的健康)」が高まるという影響方向にあった. 主観的健康感は「心身機能(一病息災的健康)」の観測変数の中でも,疾病の数,痛みの数よりも「心身機能(一病息災的健康)」により強く関連していた. キーワード:在宅高齢者,心身機能(一病息災的健康),活動(生活能力),社会参加,主観的健康感 |

Title: Health Relation Factors of Elderly People and Subjective Feelings of Health Purpose: The purpose of this research is to quantitatively clarify the effect-relations between the factors which relate to the health of elderly people at home, and also the relation between the health- relation factors and the subjective feelings of health. Method: Mail survey was conducted; questionnaires were sent to 27,472 people over 65 years old at home, living in 11 municipalities across the country. Four latent variables were extracted by factor analysis, and they were named "body function (self-rated health) ","activity","participation" and "environmental and personal" These names were based on the definitions provided by the International Classification of Functioning, Disability and Health. A health-structure model was constructed with the variables, and the direction and strength of their effects were calculated by covariance structure analysis. Results: There were 21,716 replies (79.0% response rate) and 19,416 were determined to be subjects. The main results are as follows (all of the path coefficients are statistically significant). In the effect-relations of health-constituent factors, "participation" activates "activity" and enhances "body function (self-rated health). " In the relation between "body function (self-rated health) " and the observed variables, the subjective feelings of health are more strongly related to "body function (self-rated health) " than the number of diseases or pain.
