|
今日の産科領域では超音波画像装置が急速に普及し,口唇裂・口蓋裂の出生前診断を可能とした.また,治療領域においても手術術式の発展は目覚しく,瘢痕の目立たない形態を獲得できるようになった.これらのことが影響し,口唇裂・口蓋裂の出生前告知は急速に拡大している.しかし,診断・告知する診療科と治療に関わる診療科が異なるためにおこる連携の不備や,本疾患の症例数自体が少ないために経験が積み重ねられないなど医療者側の問題の多くは未解決のまま,母親への告知が先行している現実がある. 本論文は,母親が抱えている問題と医療者側の問題を踏まえた上で,告知と受容支援をセットとする考えを基本に,母親に対する支援モデルの必要性とその概要について論じた.さらに,そのモデルを実践するために必要な看護支援マニュアルおよび支援実施対策を提案した. |

Prenatal diagnosis of cleft lip and cleft palate has been made possible owing to ultrasound imaging devices which have rapidly come into wide use in obstetric departments. Also, in the area of medical treatment of the disease, a remarkable development in the method of operation has allowed for inconspicuous cicatrix to be left on the patient. In consequence of the existing circumstances, the habit of prenatal notification of cleft lip and palate has increasingly expanded. On the part of medical staff, however, there are many problems left unsolved; they are such problems as caused by poor coordination between the department in charge of diagnosing and notification and that in charge of treatment, besides the insufficiency of accumulated experience due to the scarcity of the cases of the disease. And yet the notification to mothers alone antecedes the situation. The present paper discusses the necessity of supporting models for mothers to be informed prenatally of their babies having cleft lips and palates in the light of inseparability of notification and acceptance. It also discusses the general idea of the models based on the problems confronting the mothers and on the issues facing medical personnel. Furthermore, a necessary care support manual and a schedule for care practice are proposed. |
