| 医療技術の目覚しい発達により解決可能な健康問題が増え,看護実習も健康問題解決志向である看護過程の習得を重視している.しかし近年,老いや死に対峙する高齢者や終末期患者に対する看護では,医療技術を駆使した問題解決よりも人間存在としての患者の苦しみに対処するヒューマンケアが重要であることが認識されるようになった.J.ワトソンらは,新しい看護のパラダイムとして「看護者」という人間が「患者」という人間の苦悩に「共に関与するもの」として関わるトランスパーソナルな関係形成を提唱している. 本研究は,看護学生が老人施設での実習において医療技術では解決不可能な問題を抱えた患者とどのように人間関係を形成していくか,それを通してどのようにヒューマンケアを学習していくかを明らかにすることを目的とする.実習場面に指導者として参加観察した後,学生に面接を行ない,その逐語記録を資料として,患者--看護者の人間関係形成のプロセスをワトソンの記述的現象学的方法を用いて分析し,その構造化を試みた.その結果,学生と患者は「出会い」「模索と葛藤」「可能性の発見(転換点)」「トランスパーソナルな関係」の4段階を経てヒューマニスチックな人間関係を形成していくことが明らかになった.学生は挫折や葛藤を繰り返しながら人間関係形成の接点をつかみ,患者の真のニーズに触れる瞬間を得,それを転換点としてフィーリングの交換が活発となり,ヒューマンケアに変容した.そして実習の終了頃には,双方の間にトランスパーソナルな関係を築くことが可能となる.高齢者,終末期の患者においては個々の問題解決よりも,看護者と患者がそれぞれ一人の人間として全人的存在に影響を与え合い,トランスパーソナルなヒューマンケアの中で共に成熟するものであった.それは看護教育の視点に大きな示唆を与えるものと言える. | ||||||||||

| Along with the remarkable development of modern medical technology, curative factors have made it possible to solve many health problems. During clinical nursing training, students are expected to learn health problem solving processes. Recently, however, in nursing care for deteriorated elderly, incurable chronic or terminally ill patients, a different framework which posits nursing as a caring-healing system rather than one staying within the traditional cure oriented system has developed. Watson proposed that we should change the concept and paradigm of nursing based on a human care process which is led by a transpersonal relationship with clients and nurses.The purpose of this study is to clarify how nursing students can learn to develop human caring processes with elderly patients who have incurable, chronic disease during training practice. Interview records of students were transcribed and analyzed by Watson's descriptive and phenomenological analysis method. As a result, we found four core categories: "confusion at beginning", "conflict: trial and error", "to find possibilities: turning points" and "transpersonal relationships". Students went back and forth along these steps and finally established transpersonal relationships with patients which brought both students and patients to a mature stage of personality in their life history. | ||||||||||
