| 終末期看護実習において,死生観構築への働きかけとして,実際の体験や,カンファレンスなどを通して考えさせるアプローチが必要である.当学科では,「全人的苦痛の理解と尊厳性に対する援助を学び,死生観を構築すること」を目標にして終末期看護実習を行っている.そこで今回,終末期看護実習において,教育が意図している学生の死生観構築と共感性について把握するために,スピリチュアルケアに対する理解と緩和援助を中心にした実習内容の修得度評価から,死生観構築と共感性の関連について調査し,効果的な看護教育と指導について検討した. 方法:実習到達度評価は実習終了後のレポート評価も含み,学生40名を対象に,A(5)・B(4)・C(3)・D(2)の4段階評価とし,80点満点で得点化した.評価項目と得点は(1)全人的苦痛の理解と援助(40点) (2)対象の価値観・人生観を尊重した日常生活援助(25点) (3)死生観と終末期看護観の構築(15点).共感性は,角田の共感経験尺度により調査した.分析は,到達度評価得点と共感性の間の相関および,レポートの死と生の意味,看護実践に関して行った. 結果:実習到達度評価得点の平均はそれぞれ,(1)36.56点,(2)21.72点,(3)13.50点であった.共感性については,共有経験尺度得点42.80点,共有不全経験尺度得点27.70点であった.実習到達度と共感経験尺度評価の間において,6項目中5項目には相関関係は認めず,死生観と終末期看護観の構築と共有経験尺度の間に小さい相関(r=0.312, p=0.050)の傾向を認めた.レポート内容は,全人的な苦痛に向き合うことの困難さ,対象の死生観や人生観を受けとめるためのコミュニケーションの重要性が述べられており,実習での体験が死生観構築の発展に良い影響をもたらしていた.今回の調査から,実際的な生活援助を通した死生観の構築と共感性の育成の必要性が示唆された. | ||||||||||

| In clinical practice of terminal nursing care an approach to establish students' attitudes toward death is needed. We investigated the relationship between attitudes toward death and empathy. Method: An achievement rating for clinical practice of terminal nursing care was performed on 40 students. Rating items included (1) The understanding and support of patients' total pain (40 points), (2) Daily life support with respect for patients' sense of value and life philosophy (25 points), and (3) The establishment of attitudes toward death and terminal nursing care (15 points). Empathy was evaluated with Kakuta's empathic experience scale. Analysis was made for correlation between achievement rating and empathy, attitudes toward death, and practical nursing. Results: The mean achievement rating was (1) 36.56 points, (2) 21.72 points, and (3) 13.50 points, respectively. For empathy, scale of sharing experience was 42.80 points and scale of insufficient sharing experience was 27.70 points. A small correlation was identified between the establishment of attitudes toward death and empathy(r=0.312, p=0.050). Students reported difficulty in facing patients' total pain and the importance of communication to understand patients' attitudes toward death and life philosophies. Experiences in clinical practice of terminal nursing care had a positive effect on the establishment of students' attitudes toward death. | ||||||||||
