本研究は,医療的ケアを必要とする子どもが在籍する養護学校の宿泊を伴う校外学習で,宿泊前後 の養護教諭の記録を対象とし,養護教諭がコーディネートした事例を,宿泊学習についてのアセスメント(ケア・ニーズ,ニーズへの計画,計画の実行に当たっての留意点),ケア・コーディネーション(アセスメント,ケア・コーディネーションの実施,生徒の実態と評価)という分析の枠組みに基づいて分析を行った.旅行参加にあたっては,生徒のニーズとニーズへの計画を立案後,養護教諭・担任・部主事の3者が生徒を取り巻くケアの主軸となり計画を実行していた.今回は看護師配置の導入期であり,看護師の役割の明確化や連携への配慮が必要であった.養護教諭独自の機能としては,主軸の三者の中で,ケアシステムが中心となるコーディネーター的な役割を担っていた.しかし養護教諭は実施中はコーディネーションを意識して行っていなかった.今後は,(1)コーディネーターとしての位置づけ (2)コーディネーション能力を身につけるための研修の必要性が,養護教諭への課題として示唆された. |

Our research focused on a Yogo teacher who undertook placement at a residential school for mentally and physically handicapped students who required medical care. We carried out a field study, performing a before and after study of the work this teacher coordinated at the school. We used an analytical framework to record the teacher's assessment of the students' needs and the teachers plans for coordination of care, including overnight educational trips made by the students. We looked at the initial needs assessment, how this related to the actual conditions on site and how the plan was executed in terms of the original planning. The school's strategy for care provision centered on performing 3 roles in order to support the students: a Yogo teacher, a classroom teacher and a head of department. The Yogo teacher's primary function was taking responsibility for the coordination of the system of care. The planning and active responsibilities included liaising with parents, overseeing provision of medical care and general guidance. The Yogo teacher also had to take into consideration the provision of nursing care; both coordinating the nurses' deployment and making sure they understood their roles clearly. We believe our research shows that it is advisable to have a Yogo teacher who has a specific responsibility to act as a coordinator. We also believe specialist training is essential in order to develop their ability to coordinate effectively. |
