| 本研究は,心筋梗塞,あるいは心不全の急性期の治療後の患者のQOLとSOCの関連を明らかにすることである.自己記入式QOL質問表(QUIK)とSOCスケールを用いて,心筋梗塞14名,心不全18名の患者群32名と,健常者37名を対象群として調査し,以下の結果を得た.1)患者群と健常群のSOCスコアに差は認めなかった.QOL良好群と不良群を比較した場合,健常群ではSOCスコアに差は認めなかったが,患者群ではQOL良好群のSOCスコアが有意に高かった.2)QOLとSOCは有意な相関を認めたが,患者群と健常群では相関する尺度に違いを認めた.SOCは患者群では生活目標に,健常群では情緒適応と生活目標に有意な相関を認めた.3)SOC高群と低群を比較すると,SOC低群でQOLは劣悪であったが,患者群と健常群では有意差のある尺度に違いを認めた.患者群では対人関係と生活目標,健常群では情緒適応に有意差を認めた.以上より,心疾患患者におけるSOCは,QOLに直接的に影響を与える因子であることが示唆された.SOCは特にQOLの生活目標の尺度と関連があることから,生活目標を再構築する看護介入を行うことが,QOLを高めるばかりでなく,SOCの回復もはかる可能性が示唆された. | ||||||||

| This study investigated the relationship between quality of life (QOL) and sense of coherence (SOC) in acute cardiac disease patients during rehabilitation. Thirty-two patients (14 acute myocardial infarction, 18 acute heart failure) were compared with 37 randomly selected healthy controls using the QOL questionnaire (QUIK) and SOC score. The results were as follows. (1) SOC scores showed no statistically significant difference between patients and controls. In a sub-analysis, the SOC scores of patients with good QOL were significantly higher than those with poor QOL, while controls showed similar SOC scores in both good and poor QOL groups. (2) SOC was significantly correlated with QOL, but the dimensions of QUIK which correlated were different in patients and controls. In patients,SOC correlated with attitude toward life, while in controls, the correlation was with emotional adjustment and attitude toward life. (3) In comparing groups with high and low SOC scores, those with low SOC scores showed poor QOL, however, the QUIK dimensions that correlated were different for patients and controls. Human relationship and attitude toward life showed significant differences in patients with low SOC scores, while controls showed a significant difference in emotional adjustment. These data suggested that SOC had a direct influence on QOL in patients with acute cardiac disease. Because SOC had such a close relationship with attitude toward life, it is suggested that nursing interventions to help patients improve their attitudes toward life would increase QOL, in addition to improving SOC. | ||||||||
