|
意識障害のある患者を担当した学生の戸惑いや患者への歪んだ見方が問題意識となり,3週間の成人看護学実習の中で,口腔ケアを手がかりに患者に大切に出会える視点の育成を試みた.7名の学生の学びの過程を,実習初期と最終日の口腔ケア時におけるプロセスレコードの記述内容と学生へのインタビューの記録から分析した.結果,実習初期から,学生達は口腔ケアを意識障害患者に添った実践にしようと努力していた.そして,その実践は実習が進むにつれて,患者の立場を尊重したケアに高められていた.学生の学びの特徴から,I型.他者の意見を参考に学習を深めていく,II型.自分で学習を組み立てていく,III型.学生のみの実践では困難を要した,の3タイプに分けることができた.また,「声かけができる」「現象の観察ができる」「ルチーンでの関わりができる」「学生自身の気持ちを表出できる」「学生自身が感じた感覚を伝達できる」「他者の意見を参考に行動化できる」「行為による観察ができる」「反応を患者の立場から捉えられる」「患者の立場にたって言動ができる」「工夫した関わりができる」の10項目の学生の学習課題とそれに合わせた教師の実習指導上の課題が明らかになった. |

Nursing Students are often confused to at how to administer oral care to comatose patients. So, a 3 week training course for nursing students to take care of patients seriously, based on oral care, was developed. Seven students were analyzed using process records and an interview. Analysis was carried out during the initial stage and the final day of training. As a result, the students tried to practice oral care for a comatose patient from the early stages of training. And the practice resulted in better care as the students learned to better respect the situation of the patient as training progressed. Then, based on student performance, the students were divided into three types. I Students who could refer to the opinion of the others and deepen study, II Students who could study by themselves, III Students who couldn't practice easily by themselves. Moreover, the connection between the teacher's training subject instructions and the below 10 items became clear: "Talking to the patient", "Observing a phenomenon", "Dealing with a patient as a routine", "Expressing own feelings", "Communicating the feelings which the students felt", "Acting based on the opinion of others", "Observing by act", "Understanding from a patient position to a reaction", "Acting in a patient position" and "Doing devised relation". |
