| 近年, 在宅医療・看護が推進されたことにより複数の疾患をもち, 医療機関から多種類の処方を受け, 多くの診療科を受診して薬物療法を行っている在宅高齢者が増加している.そこで, 在宅高齢者における多剤併用中の服薬の実態を調査し, その看護ニーズを明らかにすることを目的で本研究を行った.その結果, (1)1日に平均7種類の服用がなされていた, (2)要介護認定「要支援」の在宅高齢者の服薬の実態は, 自己管理に任されている, (3)服薬コンプライアンスが十分ではないことや, 処方薬のみでなく一一投薬, 漢方薬を含めた服薬の実態把握や服薬マネージメントがなされていない, (4)在宅高齢者の生活を支える主治医問の連携や看護婦・士, 薬剤師との連携が不十分で, 服薬支援システムが構築されていないということが明らかになった.これらの服薬の問題から, 在宅高齢者の看護度を上げないためには, 在宅高齢者への処方の改善と服薬の支援システムの構築が必要であることが示唆された. | ||||||||

| Recently, due to improved medical treatments and nursing at home, the number of elderly nursing home residents with more than one disease, who consult several medical clinics and receive numerous and various kinds of drugs, is increasing. This study investigates the actual state of medication for elderly nursing home residents and elucidates the needs of nursing care. The following results were obtained :(1) Rather high daily dosage of various kinds of drugs is given. (2) Elderly nursing home residents, even those recognized as in need of assistance, are often self administered. (3)Compliance in taking medication is low. The actual state of compliance for taking not only prescribed drugs but also OTC drugs, including Chinese medicine, is not clear and drug monitoring is not done often enough. (4) The cooperation among doctors, nurses and pharmacists is missing and there is no support system. In order for the elderly to maintain ADL capabilities at home, it is necessary to improve method of medication and construct a support system. | ||||||||
