|
理学療法教育における臨床実習は, 理学療法士作業療法士学校養成施設指定規則により専門必修科目53単位のうち18単位を占め, 大変重要視されており, その意義は大きい.臨床実習においては, 教育目標, 教育評価, 指導方法等の問題が指摘され, また本学本学科が学内実習の一環として行っている隣接する医科大学附属病院での臨床基礎実習1,IIも類似した問題を抱えている.本研究の目的は3年次に行う患者評価を主体とした臨床基礎実習1において, 学生が何に躓いているのかを把握, 分析し, 今までの臨床基礎実習1の教育目標, 指導内容, 及び今後の改善策を検討することにある.そこで我々は2クールに分けて行われる臨床基礎実習1の各クール終了後に, 我々が作成した実習評価表を用いて学生自身による自己評価及び教員による学生評価を実施し, 算出した評定平均値等から到達度の低い行動目標項目を抽出し, その評定理由等の具体的内容を検討した.第1クールで到達度の低かった項目のうち, 学生と教員で一致した項目は「評価結果の統合と解釈ができ, 問題点を整理できる」, 「基礎医学知識」の2項目であった.またこの2項目は, 学生では1・2クールとも到達度が低かった.具体的内容について, 第1クールでは, 多くの学生は各項目の目標到達の困難さを漠然と記載していたが, 第2クールでは, 目標到達の困難さをより具体的に記載していた.学生が記載していた具体的内容の変化から, 実践を通しての理学療法評価過程の理解とともに, 自己の問題点が明確化されたものと思われた.「結果の統合と解釈・問題点の整理」では, 機能障害と能力障害の因果関係の理解で躓く学生も多く, 問題点の階層性, 残存能力の発掘, 回復能力の推測までは困難と思われた.1・2クールとも到達度の低い項目は, 指導方法の見直しとともに, 臨床実践能力レベルに対応した到達目標を検討する必要があると思われた. |

The purpose of this study was to identify the problems faced by physical therapy students and improve the achievement of goals in preliminary clinical practice. Thirty-six third year students in the physical therapy program and their six supervisors participated in the study. The clinical practice was divided into two six-week periods. After each session the students completed a self-evaluation and were also evaluated by their supervisors. In the evaluations, students were graded on the achievement of their goals and their descriptions of the reasons for their judgements. The results were as follows : (1) Goals that were difficult for students to achieve were the integration and interpretation of patient evaluation results. Students also experienced difficulty due to a lack of fundamental medical knowledge. (2) Students were able to recognize their difficulties more clearly after the second clinical practice. It was difficult for most students to understand the relationships between impairment and disability levels when integrating and interpreting evaluation results. Moreover, they had difficulty in prior problems, evaluating residual functions and giving prognoses. These results suggest that it is necessary to reconsider the focus/goals of clinical practice instruction in order to improve students' skills. |
