本調査においては, 知的障害者に対する在宅介護者の介護負担度を探るために, 介護者の負担感を反定量的に評価する指標として開発されたCostofCareIndex(CCI)を導入し, 岡山県内の各福祉作業所で働く知的障害者の保護者の方(85名)と自宅で老人の介護をしている在宅介護者(270名)を調査対象にして, その負担感の違いに焦点を当て両者の差異を比較検討した.その単純集計の結果から, (1)社会的制約, (2)心身の健康, (3)意欲, (4)不愉快, (5)経済的負担の5つについて統計的手法で比較したところ, すべてにおいて老人の在宅介護者の方が, 知的障害者の在宅介護者よりも負担を感じていることが明らかになった.これは, 知的障害者の在宅介護者の方は, 被介護者である知的障害者が日中は福祉作業所で働いているため, 介護の必要性がないが, 老人の在宅介護者の方は, 一日の大半を介護に費やさなければならないという時間的問題及び被介護者である高齢障害者のADLの低下が要因と考えられる.そこで対策として, 高齢障害者の在宅介護者の方は, ショートステイ, デイサービス, ホームヘルプサービスなどの社会的資源を活用し, 介護の負担を減らすことが望まれる.また, 在宅知的障害者の介護者自身が感じている問題として最も多かったものは, 健康についての将来への不安で, 半数近くの人が不安を感じていることが明らかになった. |

The home caregivers' physical and mental burden in caring for the intellectually disabled was examined using a questionnaire, modified Japanese version of the Cost of Care Index. The questionnaire was sent to 85 caregivers. Differences in workload between the caregivers for the intellectually disabled and those for the impaired elderly from previous reports were analyzed by statistical method and revealed the following. Generally speaking, the workload of the caregivers of the intellectually disabled was less than that for the caregivers of the impaired elderly. The intellectually disabled work in a sheltered work institution, during which care-givers are relieved from the burden of care. In contrast, caregivers for the impaired elderly must give care all day long. Therefore, programs for short stay and day care in medical welfare institutions for the impaired elderly are needed. The greatest concern of the caregivers of the intellectually disabled was health problems in the future. |
