| イギリスの要介護老人のためのコミュニティケア政策の構造と手続は, ケアマネジメントのシステムを発展させてきた.ケアマネジメントにおいては, ケアアセスメントが非常に重要な役割を担っている.アセスメントによって, ケアの的確性と費用効果が促進されるからである.アセスメントにつづいて策定されるケアプランは, 一般医, ソーシャルワーカー, 地区看護婦, ケアワーカーなどのチームワークを必須としている.同時に, コミュニティケア政策は費用問題を浮かび上がらせている.したがって, 多くの社会保障制度がそれを十分とはいえないにしても, 支えているという実態である. | ||||||||

| The structure and procedure of Community Care Policy for frail elderly people in Britain have developed care management system. In care management care assessment has very important roles. It gives accuracy of care and cost benefit effects. Care plan after the care assessment needs good team work for many professionals, for example general practitioner, social worker, district nurse and care worker. At the same time recent Policy needs the problem of cost of community care. Therefore, many social security schemes support this Policy, even though they are not complete. | ||||||||
