全失語の1例と重度ブローカ失語の2例に対するジェスチャー訓練の結果から, 重度失語症者に対するジェスチャー訓練の適用の条件について考察した.ブローカ失語の2例は観念運動失行を伴っていたのに対し, 全失語の1例は観念運動失行と観念失行の合併が疑われた.いずれの症例も訓練前にはスピーチのみならずジェスチャーによる意思伝達も不能であった.動作絵およびジェスチャーで表すという課題を一定期間行った結果, 全失語の症例はいくつかの動作は可能となったが成績に伸びが見られず, 実用的なジェスチャーは獲得されなかった.これに対し, ブローカ失語の2症例はいずれの課題でもコンスタントな成績の向上が見られたうえ動作の質的な変化も現れ, 実用的なジェスチャー能力が獲得された.このことよりジェスチャー訓練で効果を上げるためにはジェスチャー障害が観念運動失行のみに由来する問題であることと, 言語理解力および動作性知能が一定レベル以上に保持されていることが必要であることが示唆された. |

The gesture training process of one global and two Broca aphasic patients was reported, and factors of aquisition of gesture perfomance were discussed. One global aphasic patient was suspected to be accompanied with ideational and ideomotor apraxia. Two Broca aphasic patients were accompanied with ideomotor apraxia. All cases did not have communicative ability of both speech and gesture before training. The task required describing action or object pictures by gesture. Such task was undertaken for each patient for several months. The global aphasic patient showed only limited improvement reaching plateau state immediately and could not acquire pragmatic gesture ability. Contrarily two Broca aphasic patients showed improvement continuously and acquired pragmatic gesture ability. It was suggested that gesture training can be effective when gesture disturbance results from only ideomotor apraxia, and the ability of verbal recognition and performance IQ is kept on a certain level. |
