ICD-11導入に向けた準備が進められる中，高精度なコーディング結果を導くための環境整備の一つに，コーダーのための学習支援の充実がある．2024年国際統計分類委員会が実施したアンケートでは，我々が開発したガイドシステムのように日本語で ICD-11コードを検索できるシステムが必要だと100名が回答した．そこで，そのガイドシステムの使用がコーディング結果に与える影響を調査し，ガイドシステムの改善点を導出するために研究を実施した．医療機関で ICD コーディング業務を担った経験のある診療情報管理士37名に対し，ガイドシステム使用前・後にコーディングテスト30問を実施し，そのデータを用いて，誤答の傾向を分析した．その結果，ガイドシステム使用前・後において解答率・正答率・ステム正答率の中央値が全て向上したことから，ガイドシステムの使用は，ある一定の効果につながったのではないかと推察する．しかし，多くのテスト問題において正答率が低いため，誤答パターンを13種類のカテゴリに分類し，各カテゴリに関するガイドシステムの改善点を導出した．今後は，ICD-11自動コーディングシステムの動向を把握しつつ，本研究で明らかとなった誤答パターンに応じた教材の個別化や，外因コードを含む高難度領域への特化，エクステンションコード用解剖学アトラスの実現，外因検索アプリ開発等，効率的で高精度な ICD-11コーディングが可能な学習支援システムへと改良していきたい． |

As preparations for the implementation of ICD-11 advance, one crucial aspect of establishing an environment conducive to accurate coding outcomes is the enhancement of educational support for coders. Therefore, we conducted a study to investigate the impact of our developed educational support tool, the“Guide System,”on coding outcomes, and to identify areas for its improvement. The participants were 37 certified health information managers with prior experience in ICD coding at medical institutions. Each participant completed a set of 30 coding test questions both before and after using the Guide System. The collected data were analyzed to identify patterns in incorrect responses. As a result, the median values for response rate, accuracy rate, and stem accuracy rate all improved following the use of the Guide System. These findings suggest that the use of the Guide System may have contributed to a certain degree of effectiveness. However, since many of coding test questions demonstrated low accuracy rates, future developments should focus on enhancing the learning support system by individualizing educational materials based on error patterns and targeting high-difficulty areas, including external causes codes, to enable more efficient and accurate ICD-11 coding. |
