我が国では，2040年85歳以上人口の増加が見込まれている．地域別にみると，85歳以上人口が増加する地域もあれば減少する地域もある．85歳以上人口の増加や減少は，医療需要とくに在宅医療に影響を与えることになる．そこで，本研究は，2040年の岡山県二次保健医療圏別の在宅医療のニーズを把握することを目的とした．地域別のニーズを把握するために，NDB（匿名医療保険等関連情報データベース）オープンデータと地域の性年齢別人口データを用いて在宅医療の需要予測を行った．分析の結果，岡山県内の在宅医療の需要は，県南東部保健医療圏を除き，実績値よりも需要推計値の方が高く，在宅医療のニーズを満たせていないことが分かった．そして，2040年の需要予測の結果，岡山県全体の在宅医療は5％増加することが明らかとなった．さらに，在宅患者訪問診療料を同一建物居住者とそれ以外で比較すると，県南部では同一建物居住者の件数が多い．それに対して県北部では，同一建物居住者以外の件数が多いことが分かった．在宅医療の将来予測を通して，在宅医療の継続的提供のためには，家庭内の介護力，家族の支援に加え，地域特性を考慮した医療提供体制の整備が必要であり，市町村において地域の在宅医療を含めた地域包括ケアの作り方の議論が求められている． |

In Japan, the population aged 85 and over is expected to increase in 2040, and the increase or decrease in the population of 85 years and older will affect home medical care. The purpose of this study was to identify home healthcare needs by secondary medical care area in Okayama Prefecture in 2040. To identify needs by region, home medical care demand was projected using NDB (National Database of Health Insurance Claims) open data and population data by region. The results of the analysis indicated that the estimated demand for home medical care in Okayama Prefecture is not being met; the demand projection for 2040 reveals a 5% increase in the overall demand for home medical care in Okayama Prefecture. Furthermore, a comparison of home visitation fees between residents of the same building and other residents shows that the southern part of the prefecture has more home visitation fees for residents of the same building. On the other hand, the northern part of the prefecture has more home-visit fees for residents other than those living in the same building. For the continuous provision of home medical care, it is necessary to develop a medical care system that takes into account regional characteristics in addition to home care capabilities and family support. Discussions on how to create a community-based care system, including local home medical care, are needed in communities. |
