| 近年，障害者支援において意思決定支援の重要性が取り上げられている．本研究は，重症心身障害児（者）（以下，重症児（者）とする）をケアしている看護師は言語理解が困難とされる重症児（者）の意思をどのように汲み取り確認方法としているのかを明らかにすることを目的とした．看護師9名を対象とし，半構造化面接を行い質的帰納的に分析した．その結果，【重症児（者）の普段と異なる生理的な反応から意思の表出を捉える】，【普段と異なる表情や声の変化から意思の表出を捉える】，【重症児（者）が表す複数の変化から意思の表出を捉える】，【相互作用を通して反応を得る技を駆使し意思の表出を捉える】，【他者の意思の確認方法を学び取る】，【自らの確認方法を断定しない姿勢を持つ】，【重症児（者）の意思を知る努力を持続する】の7カテゴリーが抽出された．重症児（者）をケアしている看護師は，重症児（者）と真摯に向き合う態度を基盤に持ち，観察により意思の表出方法の把握を行い，普段と異なる数値や部位の変化から意思を汲み取っていた．そして，その汲み取った内容を重症児（者）にフィードバックし，さらに反応を読み取るといった相互作用を行う技を駆使し意思の確認を実践していることが示された． | ||||||||||

| In recent years, the importance of decision-making support in assisting persons with disabilities has been addressed. The purpose of this study was to clarify how nurses who care for patients with SMID (severe motor and intellectual disabilities) use a method to understand and confirm the intentions of patients with SMID who are considered to have difficulty understanding language. Semi-structured interviews were conducted with 9 nurses and analyzed qualitatively inductively. As a result, 7 categories were extracted, including [capturing expressions of intention from physiological reactions of patients with SMID that differ from the usual], [capturing the expression of intention by using techniques to obtain a response through interaction], and [maintaining efforts to know the intentions of patients with SMID]. Nurses caring for patients with SMID had a sincere attitude toward the patients with SMID as their foundation, and they grasped the ways of expressing their intentions through observation, and were able to grasp their intentions from unusual numerical values and changes in parts of the body. The nurses also practiced the confirmation of the intention by using interaction techniques such as feeding back the obtained information to patients with SMID and reading his or her responses. | ||||||||||
