The subjects were four persons with aphasia and their families. Family members’ evaluations of language function were lower or higher than the evaluations of ST. It became clear that the evaluations by family members were based on the communication successes or failures in important situations. These requirements differed by communication partners, i.e., family or ST, and by the setting, i.e., home or therapy room. The word-finding difficulty was easily conceived as an aphasic symptom. However, the simplified speech was conceived as a symptom of dementia, and semantic comprehension disorder in sensory aphasia was not conceived as an aphasic symptom. Cognitive disorders were not evaluated as cognitive disorders when the family members judged the aphasic person’s performances as meaningful.
